炮山甲联合化痰消瘀方治疗慢性萎缩性胃炎伴异型增生疗效观察

摘　　要：[目的]观察炮山甲联合化痰消瘀方治疗慢性萎缩性胃炎(CAG)伴异型增生临床疗效。[方法]入选经胃镜及病理诊断为CAG伴异型增生的患者60例。按随机数字表法分为对照组和治疗组各30例.对照组采用化痰消瘀方加减治疗,疗程为6个月;治疗组30例在对照组治疗的基础上在疗程的前3个月加用炮山甲颗粒剂。治疗后比较2组患者的临床总疗效、胃镜病理检查胃黏膜萎缩和异型增生的病理变化和积分情况。[结果]治疗后,治疗组临床总有效率显著高于对照组(P<0.05);2组胃黏膜萎缩与异型增生的病理变化及积分较治疗前均显著降低(均P<0.05),且治疗组显著低于对照组(均P<0.05)。[结论]炮山甲联合化痰消瘀方治疗CAG伴异型增生疗效显著,其临床疗效优于单纯使用化痰消瘀方。[Objective]To observe the clinical effect of stir-baked Squama Manitis combined with Huatan Xiaoyu Decoction on chronic atrophic gastritis with dysplasia.[Methods]Sixty patients diagnosed as chronic atrophic gastritis with dysplasia by endoscopic examination and gastroscopy biopsy were eligible for enrolment.Subjects were randomly assigned(1:1)to control group and treatment group to receive a 6-month treatment course.Control group was given Huatan Xiaoyu Decoction for 6 months,and treatment group was given Pao Shan Jia Granule and Huatan Xiaoyu Decoction at first 3 months and Huatan Xiaoyu Decoction at the following 3 months.After administration,the clinical efficacy,pathological changes and scores of gastric mucosal atrophy and dysplasia by endoscopic examination and gastroscopy biopsy were compared between the two groups.[Results]After therapy,the total effective rate in treatment group was significantly higher than that in control group(P<0.05).Pathological examination showed improvement of gastric mucosal atrophy and dysplasia in both groups after course,with scores after treatment significantly lower than those before treatment(P<0.05),and the scores in treatment group was significantly lower than those in control group(P<0.05).[Conclusion]Stir-baked Squama Manitis combined with Huatan Xiaoyu Decoction is effective in treating chronic atrophic gastritis with dysplasia,and its clinical efficacy is superior to that of Huatan Xiaoyu Decoction alone.
