改良胸大肌岛状肌皮瓣与传统皮瓣用于舌癌术后修复的效果比较

摘　　要：目的探讨改良胸大肌岛状肌皮瓣、传统皮瓣用于舌癌术后修复的实际价值.方法选取郑州大学第一附属医院2016年9月至2018年6月48例舌癌患者,随机分为研究组和对照组,每组24例.研究组患者行连续整块切除手术后接受改良胸大肌岛状肌皮瓣修复手术,对照组患者行连续整块切除手术后实施传统皮瓣修复术,比较两组手术情况、修复情况和不良反应发生情况.结果研究组手术时间与对照组比较,差异未见统计学意义(P>0.05),但研究组术后愈合时间少于对照组(P<0.05);研究组修复总有效率(95.83%)高于对照组(70.83%,P<0.05);研究组术后上臂运动障碍、感染、皮瓣移植后臃肿发生率(0、4.17%、4.17%)均低于对照组(29.17%、100.00%、25.00%),差异均有统计学意义(P均<0.05).结论应用改良胸大肌岛状肌皮瓣修复舌癌术后患者,效果满意.Objective To investigate the practical value of modified pectoralis major myocutaneous musculocutaneous flap and traditional flap for postoperative repair of tongue cancer. Methods Forty-eight patients with tongue cancer in the First Affiliated Hospital of Zhengzhou University from September 2016 to June 2018 were randomly divided into study group and control group, with 24 cases in each group. The patients in the study group underwent continuous resection of the pectoralis major muscle flap after continuous mass excision. The patients with tongue cancer in the control group underwent continuous mass resection and traditional flap repair. Surgery, repair and incidence of adverse reactions were compared between the two groups. Results The operation time of the study group was significantly lower than that of the control group ( P>0. 05 ). The healing time of the study group was significantly lower than that of the control group ( P<0. 05 ). The total effective rate of the study group (95. 83%) was significantly higher than that of the control group (70. 83%, P<0. 05). The incidence of postoperative upper arm dyskinesia, infection, and edema after flap transplantation were significantly lower in the study group (0, 4. 17%, 4. 17%, respectively) than those in the control group (29. 17%, 100. 00%, 25. 00%), the differences were statistically significant ( P all < 0. 05 ). Conclusions Modified pectoralis major myocutaneous island flap in repair for tongue cancer patients has satisfactory therapeutic effects.
