摘　　要：目的探讨复发性颅底脊索瘤的临床表现、病理特征、治疗及预后.方法回顾性分析2013年1月至2018年4月就诊于郑州大学第一附属医院的61例脊索瘤患者中12例经病理证实为复发性颅底脊索瘤患者的临床资料.结果12例患者因已行相关治疗(7例已行放射治疗),有11例患者临床表现为头痛、视功能受损等颅神经受损症状,均行手术切除肿瘤,其中5例患者肿瘤全切,余7例患者部分切除.7例已行放疗的患者病理指标Ki-67阳性率较5例未行放射治疗者明显升高.随访时间为13~85(44.67±26.73)个月,11例存活患者仍在随访中.结论复发性脊索瘤患者临床表现以颅神经损伤为主,在治疗上首选手术彻底切除肿瘤并辅助放化疗,可在一定程度上改善患者预后.Objective To investigate the clinical manifestations, pathological features, treatment and prognosis of recurrent skull base chordoma. Methods A retrospective analysis was made of the clinical data of 12 patients with recurrent skull base chordoma were confirmed by pathology of the 61 patients with chordoma who were admitted to the First Affiliated Hospital of Zhengzhou University from January 2013 to April 2018 . Results Among the 12 patients, because of the relevant treatment ( 7 patients had undergone radiotherapy), 11 patients showed symptoms of cranial nerve damage such as headache and visual function impairment, and the tumors were surgically removed. Five patients underwent total tumor resection, and the remaining 7 patients were partially resected. The positive rate of Ki-67 in 7 patients who had undergone radiotherapy was significantly higher than that in 5 patients without radiotherapy. The follow-up period was 13 to 85 (44. 67 ± 26. 73) months. Eleven surviving patients were still in the follow-up. Conclusions The main clinical manifestation of patients with recurrent chordoma is cranial nerve injury. Surgery and adjuvant chemoradiotherapy are the first choice in the treatment of recurrent chordoma, which can improve the prognosis of patients to a certain extent.
