摘  要:目的探讨缺血性脑卒中患者血清S100β及神经元特异性烯醇化酶(NSE)水平与神经缺损程度、认知障碍的相关性。方法依据简易智能状态检查量表(MMSE)评估结果,100例缺血性脑卒中患者发生认知障碍50例,无认知障碍50例。采用ELISA法检测血清S100β及NSE水平,并分析其与MMSE、美国国立卫生研究院卒中量表(NIHSS)评分的关系。结果认知障碍组与无认知障碍组患者血清S100β、NSE水平及NIHSS、MMSE评分比较,差异均有统计学意义(均P<0.05)。血清S100β、NSE水平与NIHSS评分均呈正相关(R^2=0.474、0.625,均P<0.05),与MMSE评分均呈负相关(R^2=0.491、0.608,均P<0.05)。logistic回归分析显示,血清S100β、NSE水平及NIHSS、MMSE评分均是卒中后认知障碍(PSCI)的独立影响因素(均P<0.05)。病变部位在额颞顶叶的PSCI患者MMSE评分明显低于基底核、脑干病变者(均P<0.05)。结论血清S100β及NSE水平与缺血性脑卒中后神经功能缺损、认知障碍关系密切。Objective To investigate the correlation of serum neuron specific enolase(NSE)and S100βwith degree of neurological deficit and cognitive impairment in patients with ischemic stroke.Methods According to the results of the Mini-mental State Examination(MMSE)assessment,100 patients with ischemic stroke were assigned into cognitive impairment group(50 cases)and non-cognitive impairment group(50 cases).The levels of serum NSE and S100βwere measured by ELISA;the cognitive dysfunction were evaluated with MMSE and National Institutes of Health Stroke Scale(NIHSS)scores.The correlation of serum NSE and S100βlevels with cognitive dysfunction was analyzed.Results There were significant differences in NIHSS,MMSE scores and serum S100βand NSE levels between the cognitive impairment group and the non-cognitive impairment group(P<0.05).The NIHSS score was positively correlated with serum S100βand NSE levels(R^2=0.474,0.625,both P<0.05).The MMSE score was negatively correlated with the levels of serum S100βand NSE(R^2=-0.491,-0.608,both P<0.05).Logistic regression analysis showed that NIHSS,MMSE scores and the levels of serum S100βand NSE were independent risk factors for post-stroke cognitive impairment(PSCI)(both P<0.05).The MMSE scores of PSCI patients with lesions in the parietal lobe were significantly lower than those with lesions in the basal ganglia and brainstem lesions(P<0.05).Conclusion The levels of serum NSE and S100βare closely correlated with neurological deficits and cognitive impairment in patients with ischemic stroke.
