卵巢癌患者血清人附睾蛋白4、抗缪勒管激素水平与T淋巴细胞水平的相关性研究

摘　　要：目的探讨卵巢癌患者血清人附睾蛋白4(HE4)、抗缪勒管激素(AMH)水平与T淋巴细胞水平的相关性。方法选取78例卵巢癌患者、53例卵巢良性肿瘤患者和39例健康体检者,分别作为观察组、对照1组和对照2组。采用化学发光仪检测3组受试者血清HE4和AMH水平,采用流式细胞仪检测观察组与对照1组患者T淋巴细胞亚群水平,包括CD3^+、CD4^+、CD8^+水平和CD4^+/CD8^+。相关性分析采用Pearson相关性分析。结果观察组患者HE4水平明显高于对照1组患者和对照2组受试者,AMH水平明显低于对照1组患者和对照2组受试者,差异均有统计学意义(P﹤0.01)。对照1组患者与对照2组受试者HE4和AMH水平比较,差异均无统计学意义(P﹥0.05)。观察组患者CD3^+、CD4^+水平和CD4^+/CD8^+均明显低于对照1组患者,CD8^+水平明显高于对照1组患者,差异均有统计学意义(P﹤0.01)。HE4与CD3^+、CD4^+、CD4^+/CD8^+均呈负相关(P﹤0.05),与CD8^+呈正相关(P﹤0.05);AMH与CD3^+、CD4^+、CD4^+/CD8^+均呈正相关(P﹤0.05),与CD8+均呈负相关(P﹤0.05)。结论卵巢癌患者中HE4水平较高,AMH水平较低,与T淋巴细胞存在相关性,测定其水平有助于评估患者疾病严重程度并指导临床治疗。Objective To investigate the correlation between serum human epididymis protein 4(HE4),anti-Müllerian hormone(AMH)levels and T lymphocyte levels in patients with ovarian cancer.Method A total of 78 patients with ovarian cancer,53 patients with benign ovarian tumors and 39 healthy subjects were included in the research as study group,control group 1,and control group 2.The levels of HE4 and AMH in the three groups were determined by chemiluminescent assay,besides,the levels of CD3^+,CD4^+,CD8^+,and CD4^+/CD8^+were also determined in study group and control group 1.Pearson correlation analysis was performed accordingly.Result The levels of HE4 in study group were higher than that in the control group 1 and control group 2,and the AMH level in study group was lower compared to control group 1 and 2(P<0.01).The HE4 and AMH levels in control group 1 and 2 were similar,with no significant differences noted(P>0.05).In study group,the levels of CD3^+,CD4^+and CD4^+/CD8^+were significantly lower than that in control group 1,while the CD8^+level of study group was higher than that of the control group 1(P<0.05).Negative correlation was observed for HE4 and CD3^+CD4^+,CD4^+/CD8^+levels in study group(P<0.05),while there was a positive correlation for CD8^+instead(P<0.05);AMH levels were positively correlated with CD3+,CD4+,CD4+/CD8+(P<0.05),and were negatively correlated with CD8^+(P<0.05).Conclusion The expression of HE4 is relatively high while AMH is low in patients with ovarian cancer,though both are related to T lymphocytes.Detecting the expression level would be helpful to evaluate the severity of the disease and guide clinical treatment.
