摘 要:目的探讨声门上喉癌患者STAT3的表达与手术切缘长度、术后复发及临床病理因素的关系。方法应用免疫组化SP法检测82例声门上喉癌患者病理阴性边缘标本中STAT3的表达情况。结果喉癌原发灶中STAT3的阳性表达率为83%(68/82)。手术切缘STAT3阳性表达组与阴性表达组的复发率差异有统计学意义(P<0.05)。分析手术切缘长度与术后复发率的关系,得出以STAT3表达判断肿瘤边缘安全手术边缘为5 mm。随访3年,生存分析结果显示STAT3手术切缘阴性表达组的生存率明显高于阳性表达组(P<0.05)。淋巴结转移组手术切缘STAT3的阳性表达率明显高于无淋巴结转移组(P<0.05),临床分期T3、T4组明显高于临床分期T1、T2组(P<0.05)。不同病理组织学分级的手术切缘STAT3阳性表达率差异无统计学意义(P>0.05)。结论 STAT3蛋白可作为评估手术切缘和预后的重要分子标志物。Objective To investigate the relationships between the expression of STAT3 and the length of surgical margins,postoperative recurrence and clinical pathological conditions in supraglottic carcinoma patients. Methods Immunohistochemical technique(SP) was used to detect the expression of STAT3 in the pathological negative margin specimens of 82 supraglottic carcinoma patients. Results Expressions of STAT3 were detected 83%(68/82)in primary sites of laryngeal cancer. The recurrence rates of positive expression group and the negative expression group of STAT3 in surgical margin were different(P<0.05). The safety surgical margin was 5 mm away from tumor edge judged by STAT3 expression. After three years’ follow-up, the survival analysis suggested that the negative expression of STAT3 group in surgical margin had a significantly higher survival rate than the positive expression group(P<0.05). The positive expression rate of STAT3 in surgical margin in lymph node metastasis group was significantly higher than in no lymph node metastasis group(P<0.05), and it was also significantly higher in the group of clinical stage T3 and T4 than in the group of clinical stage T1 and T2(P<0.05). The difference of positive expression rate of STAT3 group in surgical margin among different histopathologic grade had no statistical significance(P>0.05). Conclusions STAT3 protein could be used as an important molecular marker to evaluate the surgical margin and prognosis.
