耳内镜下Ⅰ型鼓室成形干湿耳手术199例疗效观察

摘　　要：目的观察耳内镜下Ⅰ型鼓室成形干湿耳手术的疗效。方法选取耳鼻喉科2018年6月~2019年6月收治的慢性化脓性中耳炎患者199例,均于干湿耳状态下进行手术治疗,根据不同术式将其纳入对照组(69例)与观察组(130例),对照组行传统耳显微镜手术,观察组行耳内镜下Ⅰ型鼓室成形术,对比其临床疗效、听力恢复情况以及并发症发生情况。结果观察组的总有效率(92.3%)高于对照组(79.7%)(P<0.05);两组术后的气导、骨导、骨气导间距对比差异有统计学意义(P<0.05);观察组的并发症发生率(3.8%)低于对照组(14.5%),差异有统计学意义(P<0.05)。结论耳内镜下Ⅰ型鼓室成形干湿耳手术的疗效显著,可有效恢复患者的听力,且并发症发生率低。Objective To observe the effect of type Ⅰ tympanoplasty for dry and wet ear under otoendoscope. Methods 199 patients with chronic suppurative otitis media admitted to the otolaryngology department of our hospital from June 2018 to June 2019 were operated on under dry and wet ear condition. According to different surgical methods,they were included in the control group( 69 cases) and the observation group( 130 cases). The control group was operated on by traditional otomicroscopy,and the observation group was operated on by type Ⅰ tympanoscopy. The clinical efficacy,hearing recovery and complications of ventricular plasty were compared. Results The total effective rate of the observation group( 92.3%) was higher than that of the control group (79.7%)(P < 0.05);the gap of air conduction,bone conduction and bone gas conduction between the two groups after operation was compared(P < 0.05);the complication rate of the observation group( 3.8%) was lower than that of the control group( 14.5%)( P < 0.05). Conclusion Type Ⅰ tympanoplasty with dry and wet ear under otoendoscopy is effective in restoring patients' hearing,and the incidence of complications is low.
