小切口囊外摘除手术治疗小瞳孔下白内障合并糖尿病患者的临床效果

摘　　要：目的探讨小切口囊外摘除手术治疗小瞳孔下白内障合并糖尿病患者的临床效果。方法选择我院收治的26例(30眼)白内障合并糖尿病、散瞳后瞳孔直径<4 mm患者为研究对象。所有患者均给予小切口囊外摘除手术和人工晶体植入手术。术后随访1个月,观察眼部及视力恢复情况。结果所有患者手术均顺利完成。所有患者瞳孔无变形,无明显眼部并发症。患者术后1 d、1周、1个月的视力分布情况均优于术前(P<0.05)。术后1个月,76.7%的患者视力≥0.3。术后1个月,患者角膜上方和中央内皮细胞损失率分别为10.9%和8.2%。结论在熟练掌握手术技巧情况下,小切口囊外摘除手术也是一种治疗复杂白内障的安全有效方法,特别适合在基层防盲手术中的开展。Objective To evaluate the effect of small incision extracapsular extraction on cataract complicated with diabetes patients with small pupil. Methods Twenty-six patients(30 eyes) with cataract complicated with diabetes mellitus and pupil diameter <4 mm after mydriasis were selected as the study objects. All patients were treated with small incision extracapsular extraction and intraocular lens implantation. The patients were followed up for 1 month to observe the recovery of eye and visual acuity. Results All patients were successfully operated. All patients had no pupil deformation and no obvious eye complications. The visual acuity distribution at 1 day, 1 week and 1 month after operation were better than that those before operation(P<0.05). One month after operation, 76.7% of the patients had visual acuity greater than or equal to 0.3. One month after operation, the loss rates of endothelial cells in upper cornea and central cornea were10.9% and 8.2% respectively. Conclusion Small incision extracapsular extraction is also a safe and effective method for complex cataract under skilled operation skills, especially suitable for basic blindness prevention surgery.
