摘　　要：目的评估改良的有晶状体眼后房型人工晶状体(ICL)植入术的临床疗效。方法前瞻性病例对照研究。该院2016年1月至2016年12月高度近视行ICL植入术64例(128只眼)随机分为两组:对照组,32例(64只眼),采用传统手术方法,即黏弹剂辅助下ICL植入术;改良组,32例(64只眼),采用无黏弹剂辅助的ICL植入术。比较两组术后视力、眼压及拱高等。结果术后视力两组差异无统计学意义(P>0.05)。术后2小时发生高眼压者改良组4只眼(6.2%),对照组8只眼(12.5%)。经前房放液后4小时改良组眼压平稳,而对照组4只眼眼压仍高(6.2%),差异有统计学意义(χ^2=10.267,P=0.025)。术后拱高两组间差异均无统计学意义(P>0.05)。结论改良后房型ICL植入术安全、有效。Objective To evaluate the clinical efficacy of modified posterior chamber implantable collamer lens (ICL) implantation in phakic eyes. Methods A prospective case-control study. The data of 128 eyes of 64 cases with high myopia who received ICL were analyzed. All cases were randomly divided into two groups.The control group, 64 eyes of 32 cases, were treated with traditional posterior chamber intraocular lens implantation assisted with viscoelastics. The modified group, 64 eyes of 32 cases, were treated with intraocular lens implantation without viscoelastics. The postoperative visual acuity, arch height and intraocular pressure of the two groups were compared. The visual acuity, the changes of central vaulting and intraocular pressure of the two groups were compared after surgery. Results There was no significant difference in visual acuity postoperatively between two groups(P>0.05). The high intraocular pressure occurred at 2 h after operation in 4 eyes (6.2%) in the modified group and 8 eyes (12.5%) in the control group. The high intraocular pressure after anterior chamber penetration remained in 4 eyes (6.2%) of the control group and no eye of modified group.The difference was statistically significant (χ^2=10.267, P=0.025). The difference in the change of central vaulting was not significant between the two groups after operation (P>0.05). Conclusion Modified posterior chamber implantable collamer lens implantation in phakic eyes is safe and effective.
