摘 要:目的对比观察同步放化疗及序贯放化疗治疗小细胞肺癌合并上腔静脉压迫综合征(superior vena cava syndrome, SVCS)的近期效果。方法选取2010年1月—2016年1月我院收治的46例小细胞肺癌合并SVCS患者,按治疗方式的不同分为同步放化疗组25例和序贯放化疗组21例。同步放化疗组同时接受放疗与EP方案化疗;序贯放化疗组先行放疗,放疗结束后两周行EP方案化疗。治疗结束后比较两组疗效、治疗期间不良反应及近期生存率。结果同步放化疗组治疗后客观有效率为88.0%高于序贯放化疗组的61.9%( P <0.05)。两组治疗期间不良反应发生率比较差异无统计学意义( P >0.05)。两组治疗结束后半年、1年、2年生存率比较差异均无统计学意义( P >0.05)。结论同步放化疗治疗小细胞肺癌合并SVCS的近期疗效优于序贯放化疗,且不良反应和近期生存率相当,可作为本病优选的治疗方式。ObjectiveTo compare the short-term effects of concurrent chemoradiotherapy and sequential chemoradiotherapy for small cell lung cancer (SCLC) with superior vena cava syndrome (SVCS).MethodsForty-six patients with SCLC complicated with SCVS who were treated in our hospital between January 2010 and January 2016 were divided into concurrent chemoradiotherapy group ( n =25) and sequential chemoradiotherapy group ( n =21).The concurrent chemoradiotherapy group received both radiotherapy and EP regimen chemotherapy;the sequential chemoradiotherapy group received radiotherapy first, and the EP regimen chemotherapy was performed two weeks after the end of radiotherapy.After treatment, the efficacy of the two groups, adverse reactions during treatment and short-term survival rate were compared.ResultsThe objective effective rate was 88.0% in the concurrent chemoradiotherapy group, which was significantly higher than that (61.9%) in the sequential chemoradiotherapy group ( P < 0.05).There was no significant difference in the incidence of adverse reactions between the two groups ( P >0.05).There were no significant differences in the survival rates between the concurrent chemoradiotherapy group and the sequential chemoradiotherapy group at six months, one year, and two years after treatment ( P >0.05).ConclusionThe short-term efficacy of concurrent chemoradiotherapy in patients with SCLC complicated with SVCS is superior to sequential chemoradiotherapy, and the adverse reactions and short-term survival rates are similar, which, therefore, may be the preferred treatment for this disease.
