摘　　要：目的系统分析缺氧诱导因子1α(HIF-1α)和血管内皮生长因子(VEGF)与结直肠癌患者临床病理特征及预后的关系。方法计算机检索PubMed、Embase、Cochrane Library、Medline、Google Scholar及SinoMed数据库。筛选文献并行质量评价后,采用RevMan5.3软件进行Meta分析。结果VEGF阳性结直肠癌患者HIF-1α阳性率高于VEGF阴性者(P<0.05)。HIF-1α的表达与肿瘤TNM分期有关(P<0.05),VEGF的表达与术前癌胚抗原水平及淋巴结转移、远处转移有关(P<0.05)。HIF-1α阳性者的无瘤生存期、总生存期均低于HIF-1α阴性者(P<0.05)。结论结直肠癌患者HIF-1α的表达可能与VEGF的表达有关,但与两者表达相关的临床病理特征不同。HIF-1α阳性的结直肠癌患者预后较差。Objective To systematically analyze the relation of hypoxia-inducible factor 1α(HIF-1α) and vascular endothelial growth factor(VEGF) with clinicophathological features and prognosis in patients with colorectal cancer.Methods A computer-based retrieval was conducted in PubMed,Embase,Cochrane Library,Medline,Google Scholar and SinoMed databases.After literature screening and quality assessment,a Meta-analysis was performed by RevMan 5.3 software.Results The positive rate of HIF-1α in patients with VEGF-positive colorectal cancer was higher than that in VEGF-negative patients( P <0.05).HIF-1α expression correlated with TNM stage of the tumor( P <0.05),VEGF expression correlated with preoperative carcinoembryonic antigen level,lymph node metastasis and distant metastasis( P <0.05).Patients with positive HIF-1α had lower disease-free survival and overall survival than those with negative HIF-1α( P <0.05).Conclusion HIF-1α expression may correlate with VEGF expression in patients with colorectal cancer,but the clinicopathological features associated with their expression are different.Patients with HIF-1α-positive colorectal cancer have a poorer prognosis.
