摘 要:目的探索T淋巴细胞亚群在早期三阴性乳腺癌新辅助化疗疗效预测中的应用。方法采用流式细胞仪检测12例行健康体检的女性(对照组)及44例三阴性乳腺癌患者(病例组)新辅助化疗前后静脉血T淋巴细胞亚群及自然杀伤(NK)细胞比例。分析临床特征与T淋巴细胞亚群及NK细胞比例的关系,并进一步分析二者对化疗疗效的影响。结果新辅助化疗前病例组患者CD3^+、CD3^+CD8^+及CD4^+CD25^+比例均高于对照组,CD8^+CD28^+比例低于对照组,差异均有统计学意义(P﹤0.05)。新辅助化疗后病例组患者CD3^+和CD3^+CD8^+比例均提高,差异均有统计学意义(P﹤0.05)。CD8^+CD28^+比例≥12.5%的三阴性乳腺癌患者疗效明显优于CD8^+CD28^+比例﹤12.5%的患者,差异有统计学意义(P=0.002)。Logistic多因素回归分析发现CD8^+CD28^+T细胞是三阴性乳腺癌患者新辅助化疗疗效的影响因素(OR=1.361,95%CI=1.015~1.825,P=0.039)。结论三阴性乳腺癌患者免疫功能紊乱,监测三阴性乳腺癌患者外周血T淋巴细胞亚群水平对于新辅助化疗疗效预测具有一定的参考价值。Objective To explore the effect of detecting T lymphocyte subsets in predicting the response to neoadjuvant chemotherapy in patients with early-stage triple-negative breast cancer (TNBC).Method Flow cytometry was utilized to detect the ratio of T lymphocyte subsets and natural killer (NK) cell in 12 cases healthy women (control group) and 44 cases patients with TNBC (study group) before and after neoadjuvant chemotherapy.The correlation between clinical characteristics and T lymphocyte subsets and NK cells was analyzed,and their impact on clinical response were further investigated.Result The proportion of CD3^+,CD3^+CD8^+ and CD4^+CD25^+ were higher,while ratio of CD8^+CD28^+ was lower significantly in study group than that of control group (P<0.05).After neoadjuvant chemotherapy patients in study group had significantly improved proportion of CD3^+ and CD3^+CD8^+(P<0.05).Higher CD8^+CD28^+ T cells of ≥ 12.5% among TNBC patients were a significant contributor to neoadjuvant chemotherapy response as compared with those with CD8^+CD28^+<12.5%(P=0.002).Logistic multivariate analysis revealed that,CD8^+CD28^+ T cells were a factor affecting the response to neoadjuvant chemotherapy (OR=1.361,95%CI=1.015~1.825,P=0.039).Conclusion The immune function of breast cancer patients is compromised.Monitoring the T lymphocyte subsets in the peripheral blood of TNBC patients is constructive for predicting the response to neoadjuvant chemotherapy.
