化疗肺癌患者自我感受负担与疾病感知、疾病不确定感、心理弹性的关系

摘　　要：目的:探讨化疗期肺癌患者自我感受负担与疾病感知、心理弹性、疾病不确定感的关系。方法:选取病理诊断为肺癌的113例住院期化疗患者为研究对象,采用疾病感知问卷简化版(BIPQ)、心理弹性量表简化版(CD-RISC10)、疾病不确定感成人量表(MUIS-A)及癌症患者自我感受负担量表(SPBCP)分别测查患者疾病感知、心理弹性、疾病不确定感及自我感受负担水平。结果:多元逐步回归分析显示,经济/家庭负担、心理/情感负担、照护负担、治疗负担及总体负担与疾病感知正向关联(β=0.09~0.45),与心理弹性负向关联(β=-0.13^-1.09);经济/家庭负担与疾病不确定感复杂性(β=0.17)、心理/情感负担与不明确性(β=0.17)、治疗负担及总体负担与不确定感总分(β=0.06、0.18)正向关联。结论:化疗期肺癌患者疾病不确定感越高,负性感知越多,患者自我感受负担可能越重;心理弹性越好,患者自我感受负担可能越轻。Objective: To explore the correlation between self-perceived burden and disease perception,uncertainty in illness and resilience in patients with lung cancer during chemotherapy.Methods: A total of 113 hospitalized chemotherapy patients with pathological diagnosis of lung cancer were enrolled as subjects.The level of disease perception,resilience,uncertainty in illness and self-perceived burden were measured with the Brief Illness Perception Questionnaire(BIPQ),10 item Connor Davidson Resilience Scale(CD RISC 10),Mishel Uncertainty in Illness Scale for Adult(MUIS-A) and Self-Perceived Burden Scale for Cancer Patients(SPBS-CP),respectively.Results: Multivariate stepwise regression analysis showed that economic/family burden,psychological/emotional burden,care burden,treatment burden and overall burden were positively correlated with disease perception(β= 0.09-0.45) and negatively correlated with resilience(β=-0.13--1.09).Meanwhile,the economic/family burden was positively correlated with the complexity of MUIS-A(β= 0.17),as the same as psychological/emotional burden with uncertainty(β= 0.17),treatment burden and total burden with the total score of MUIS-A(β= 0.06,0.18).Conclusions: The lung cancer patients during chemotherapy may be have heavier self-perception burden when they have higher uncertainty in illness and negative perception,while it may be less whey have better resilience.
