即刻根管治疗联合纤维桩核冠修复对前牙外伤冠根折患者PD、TM、SBI的影响

摘　　要：目的探讨即刻根管治疗联合纤维桩核冠修复对前牙外伤冠根折患者探诊深度(PD)、龈沟出血指数(SBI)、牙齿松动度(TM)的影响.方法选取2016年1月~2018年6月我院前牙外伤冠根折患者86例,按治疗方案不同分观察组(n=43)、对照组(n=43).对照组予以常规根管治疗联合修复术治疗,观察组予以即刻根管治疗联合纤维桩核冠修复术治疗.比较两组手术效果、手术前后PD、SBI、TM、咬k力、X线牙片对位率及牙周感染率、患者满意度.结果观察组手术成功率95.00%(38/40)、患者满意度92.50%(37/40)较对照组高,手术失败率5.00%(2/40)、牙周感染率0.00%(0/40)较对照组低,差异有统计学意义(P<0.05);术后两组PD、SBI、TM均降低,且观察组低于对照组,差异有统计学意义(P<0.05);观察组X线牙片对位率100.00%(40/40)与对照组95.12%(39/41)对比,差异无统计学意义(P>0.05).结论即刻根管治疗联合纤维桩核冠修复治疗前牙外伤冠根折效果确切,能有效改善牙周状况,复位伤牙,提高患者满意度.Objective To investigate the effects of immediate root canal therapy combined with fiber post and crown repair on the depth of probing (PD), sulcus bleeding index (SBI) and tooth looseness (TM) in patients with anterior teeth trauma and root fracture. Methods 86 patients with anterior teeth trauma and crown fracture from January 2016 to June 2018 were selected. According to the treatment plan, the observation group (n=43) and the control group (n=43). The control group was treated with conventional root canal therapy combined with repair, and the observation group was treated with immediate root canal therapy combined with fiber post and crown repair. The effects of surgery, PD, SBI, TM, bite force, X-ray tooth alignment rate, periodontal infection rate and patient satisfaction were compared between the two groups. Results The operation success rate of the observation group was 95.00%(38/40), and the patient satisfaction rate was 92.50%(37/40). The operation failure rate was 5.00%(2/40) and the periodontal infection rate was 0.00%(0/40). The difference was statistically significant (P<0.05). The PD, SBI and TM were decreased in the two groups, and the observation group was lower than the control group, the difference was statistically significant (P <0.05). The X-ray tooth alignment rate was 100.00%(40/40) compared with the control group 95.12%(39/41), the difference was not statistically significant (P >0.05). Conclusion Immediate root canal treatment combined with fiber post and core crown for the treatment of anterior teeth trauma crown root fracture is effective, can effectively improve the periodontal condition, reduce the injured teeth and improve patient satisfaction.
