eIF4E、CDK4在胃癌及胃癌前情况中的表达及意义

摘　　要：目的研究真核细胞起始因子4E(eukaryotic initiation factor 4E,eIF4E)、细胞周期蛋白依赖性激酶4(cyclin-dependent kinase 4,CDK4)在胃癌及胃癌前情况中的表达及意义。方法收集武汉科技大学附属普仁医院2016年3月至2017年12月胃黏膜活检组织及外科手术切除标本共202例,包括慢性非萎缩性胃炎23例,慢性萎缩性胃炎30例,胃黏膜低级别上皮内瘤变25例,胃黏膜高级别上皮内瘤变27例,胃癌97例(同时收集距肿瘤边缘2~3 cm的癌旁组织),用免疫组化法测定上述标本中eIF4E及CDK4的表达。结果慢性非萎缩性胃炎患者胃黏膜中均未检出eIF4E蛋白阳性表达;在胃癌前情况及胃癌中,eIF4E的表达差异显著,呈递增趋势( P <0.01),且eIF4E蛋白阳性表达率在胃癌有淋巴结转移组和进展期组分别显著高于无淋巴结转移组和早期胃癌组( P <0.01)。在胃癌癌旁癌前情况组eIF4E蛋白阳性表达率分别明显高于非癌对应组( P<0.01或P <0.05);且癌旁与相应癌灶eIF4E蛋白检测结果存在显著正相关(φ=0.343,P <0.01)。CDK4在慢性非萎缩性胃炎患者中未见阳性表达,在胃癌前情况及胃癌中其阳性表达率逐渐升高( P <0.01)。胃癌组织中eIF4E与CDK4呈显著正相关( Kappa=0.464,P <0.01),而胃癌前情况组织中eIF4E与CDK4也呈显著正相关( Kappa=0.693,P <0.01)。结论追踪胃癌前情况患者胃黏膜eIF4E蛋白表达可能有助于胃癌的早期发现。胃癌及胃癌前情况中eIF4E高表达可能导致CDK4表达水平增高,这为深入研究胃癌的发生、发展机制及寻找胃癌治疗的分子靶点提供了线索。Objective To study the expressions and significance of eukaryotic initiation factor 4E (eIF4E) and cyclin- dependent kinase 4 (CDK4) in the diagnosis of gastric carcinoma and premalignant conditions of gastric carcinoma. Methods A total of 202 gastric mucosal biopsies and surgical resection specimens were collected from Mar. 2016 to Dec. 2017 in Puren Hospital of Wuhan University of Science and Technology, including 23 cases of chronic non-atrophic gastritis, 30 cases of chronic atrophic gastritis, 25 cases of gastric mucosal low-grade intraepithelial neoplasia, 27 cases of gastric mucosal high-grade intraepithelial neoplasia and 97 cases of gastric carcinoma (including adjacent tissues of 2-3 cm from the edge of the carcinoma). The expressions of eIF4E and CDK4 in the above specimens were detected by immunohistochemistry. Results There was no positive expression of eIF4E protein in gastric mucosa of patients with chronic non- atrophic gastritis. In premalignant conditions of gastric carcinoma and gastric carcinoma, the expression of eIF4E was significantly different, showing an increasing trend ( P <0.01). Furthermore, the expressions of eIF4E protein in lymph node metastasis group and advanced group were significantly higher than that in without lymph node metastasis group and early gastric carcinoma group ( P <0.01). The positive expression rate of eIF4E protein in gastric carcinoma paracancerous atrophic gastritis group was significantly higher than that in non-cancerous corresponding group ( P < 0.01 or P <0.05). In addition, there was a significant positive correlation between the expression of eIF4E protein in the adjacent to carcinoma and the corresponding carcinoma focus (φ=0.343, P <0.01). There was no positive expression of CDK4 in gastric mucosa of patients with chronic non-atrophic gastritis, and the positive expression rate was increased gradually in premalignant conditions of gastric carcinoma and gastric carcinoma ( P <0.01).
