探讨甲硝唑缓释药膜联合盐酸米诺环素治疗重度牙周炎的临床疗效

摘　　要：目的:分析甲硝唑药膜联合盐酸米诺环素治疗中重度牙周炎的疗效。方法:选取2016年1月至2018年1月间在本院接受治疗的牙周炎患者84例,随机数字表法分为2组,对照组42例注射盐酸米诺环素,观察组42例在对照组基础上加用甲硝唑药膜,观察牙周探诊深度(PD)、牙周附着丧失(AL)、龈沟出血指数(SBI),ELISA法检测治疗前后患者龈血清中TNF-α、IL-6及IL-8的水平,治疗后第1.5个月及3个月复诊进行临床牙周检查,并取血清标本。结果:治疗后观察组与对照组患者牙周组织内IL-8、IL-6及TNF-α含量均低于治疗前,观察组又低于对照组,差异均有统计学意义(P<0.05);治疗后观察组与对照组牙周探诊深度(PD)、牙周附着指数(AL)、龈沟出血指数(SBI)均低于治疗前,观察组又低于对照组,差异均有统计学意义(P<0.05)。结论:甲硝唑药膜联合盐酸米诺环素可有效改善中重度牙周炎患者临床症状,降低血清中炎性因子含量,对患者疗效显著,且安全性较高。Objective: To analyze the efficacy of metronidazole film combined with minocycline hydrochloride in the treatment of moderate to severe periodontitis. Methods: 84 patients with periodontitis treated in our hospital from January 2016 to January 2018 were randomLy divided into two groups. 42 patients in the control group were injected minocycline hydrochloride. 42 patients in the observation group were treated with metronidazole film on the basis of the control group. The periodontal exploration depth(PD), periodontal attachment loss(AL), gingival sulcus hemorrhage index(SBI)and ELISA were observed before and after treatment. The levels of TNF-alpha, IL-6 and IL-8 in gingival serum were examined at 1.5 and 3 months after treatment, and serum samples were taken. Results: After treatment, the contents of IL-8, IL-6 and TNF-a in periodontal tissues of the patients in the observation group and the control group were lower than those before treatment, and the contents of IL-8, IL-6 and TNF-a in the observation group were lower than those in the control group(P<0.05). After treatment, the periodontal exploration depth(PD), periodontal attachment index(AL), gingival sulcus bleeding index(SBI)of the observation group and the control group were lower than those before treatment, and The difference was statistically significant(P<0.05). Conclusion: Metronidazole film combined with minocycline hydrochloride can effectively improve the clinical symptoms of patients with moderate to severe periodontitis and reduce the content of inflammatory factors in serum. It is effective and safe for patients.
