摘　　要：临终反向关怀是从患者角度阐释患者如何给予亲友反向关怀,而嵌入性灵性照护则是从照顾者视角探讨如何更好地与患者建立灵性联结,协助患者向亲友实施临终反向关怀。运用“嵌入性灵性照护”干预框架实践于安宁疗护,开展灵性需求评估、实施嵌入性灵性照护,讨论患者与照顾者的灵性联结与互动模式。实践结果表明,在患者与“天人物我”联结层面,运用“五觉”照护法嵌入灵性照护元素,有助于家属与患者建立情感互动与联结,最终使患者善终,家属善别。Reflexive hospice care is to explain how do the patients give relatives and friends reflexive care, while the embeddedness of spiritual care explores how do the embedded spiritual care establish a better spiritual connection with the patients and assist the patients to implement the reflexive hospice care to relatives and friends. Use the "embeddedness of spiritual care" intervention framework in hospice care, conduct spiritual needs assessment, implement embedded spiritual care, and discuss the spiritual link and interaction model between patients and their families. The result shows that embedding the spiritual care elements in "Five Senses" care, and acting on the connection between the patients and the "god , relationship, nature, and self" can better promote the emotional links and interactions between patients and their families, so as to make a good ending for those who are departed, a good farewell for those who are bereaved.
