2123例儿童口腔颌面部囊肿、肿瘤及瘤样病变的临床病理分析

摘　　要：目的:探讨吉林地区儿童口腔颌面部囊肿、肿瘤及瘤样病变的发病情况及病理构成特点。方法:回顾1982~2017年在吉林大学口腔医院治疗的14岁以下口腔颌面部囊肿、肿瘤及瘤样病变患者的病历资料,按照发病年龄、性别、部位、病理类型等进行统计分析。结果:收集儿童患者2 123例(男1 064例,女1 059例,男女比1∶1)。肿瘤1 114例,其中恶性肿瘤70例(6.28%),良性肿瘤539例(25.39%)。囊肿866例,瘤样病变648例。良性肿瘤中淋巴管瘤所占比例最大(34.14%);恶性肿瘤中以肉瘤最多见(32.86%);囊肿中以舌下腺囊肿最多(19.63%);瘤样病变中以脉管畸形最多(81.17%)。良性肿瘤中颜面部和颌骨的患者数量最多,各122例,其次为舌部及腮腺;恶性肿瘤最好发部位为腮腺;囊肿、瘤样病变最好发部位分别为颌骨和颜面部。结论:儿童口腔颌面部囊肿、肿瘤构成及瘤样病变分布特点与成年人不同,病理类型是治疗方案的关键因素。Objective: To analyze the prevalence and the pathological proportion of oral and maxillofacial cyst, tumor and tumor like lesions in Chinese children. Methods: A retrospective analysis was performed on the clinical data of children( under 14years old) with oral and maxillofacial cyst, tumor and tumor like lesions in Stomatological Hospital of Jilin University from 1982 to 2017. Results:2123 cases(1 064 males and 1 059 females, with a male-to-female ratio of 1:1) were included. The data included 70 cases of malignant tumors, 539 of benign tumors,866 of cyst and 648 of tumor like lesions. Lymphangioma(34.14%), sarcoma(32.86%), ranula and vascular malformation(81.17%) were the most common pathological type of benign tumor, oral and maxillofacial malignant tumor, cyst and tumor like lesions. Facial and jaw are the most common sites of benign tumors. Parotid region are the most common site for malig nant tumors. Conclusion: The maxillofacial cyst, tumor and tumor like lesion are varied with age, sex and pathological types. The pathological type is the key factor in determining the treatment plan.
