腹腔镜手术对既往有腹腔手术史结直肠癌患者的疗效影响分析

摘　　要：目的探讨腹部手术史是否会对腹腔镜治疗结直肠癌患者的临床效应产生影响。方法采用病例对照研究法,选取2016年2月—2019年3月收治的100例结直肠癌患者进行研究,观察其是否存在腹部手术史、一般资料和临床资料。分为腹腔手术史组(68例)与无腹腔手术史组(32例),比较两组的术中指标(手术时间、术中出血量、术中输血例数和标本切除长度)、术后恢复指标(肛门通气时间、术后排便时间、术后进食流质食物时间、下床活动时间和住院天数)、并发症发生率(出血、腹腔感染、切口感染、吻合口瘘、肠梗阻和深静脉血栓)以及远期疗效(局部复发、远期转移与种植转移)的差异。结果腹腔手术史组与无腹腔手术史组比较两组术中指标、术后恢复指标、并发症发生率以及远期疗效无统计学差异(P>0.05)。结论腹部手术史对腹腔镜治疗结直肠癌患者的临床效应并不明显,既往有腹部手术史的结直肠癌患者可以行腹腔镜结直肠癌切除术。OBJECTIVE To study whether history of abdominal surgery could influence the clinical effect of laparoscopic treatment of pa tients with colorectal cancer METHODS Using case-control study to select 100 cases of colorectal cancer patients admitted in target de partment into the research from May 2013 to May 2015,observe its whether there was a history of abdominal surgery,general data and clinical data. The patientsweredivided into history of abdominal surgery(68 cases)with no history of abdominal surgery (32 cases). Comparison of two groups of indicators (hand operation time,intraoperative blood loss,intraoperative blood transfusion,the number of cases,cut of specimen length),postoperative recovery index (anus gas time,postoperative defecation time,eat liquid food bed time,postoperative hospitalization days and time) and complications (bleeding,abdominal cavity infection,infection of incision, anastomotic fistula, intestinal obstruction, deep vein thrombosis) and the incidence of the long-term curative effect (local recur rence,forward transfer and planting transfer). RESULTS There was no statistic differences of ntraoperative,postoperative recovery, complications,and long-term curative effect between these two groups(P>0.05). CONCLUSION Abdominal surgery history,the clini cal effect of laparoscopic treatment of patients with colorectal cancer were not obvious,patients always has a history of abdominal surgery could laparoscopic colorectal cancer resection of colorectal cancer patients.
