REG4在直肠癌组织中的表达与患者临床病理特征及预后的关系分析

摘　　要：目的分析REG4在直肠癌组织中的表达情况,并探讨其与患者临床病理特征及预后的关系。方法收集2010年1月至2013年1月本院接诊的98例直肠癌患者的临床资料,调取患者手术标本(癌组织、癌旁正常组织),检测癌组织与癌旁正常组织的REG4表达情况,探讨癌组织REG4表达量与患者临床病理特征的关系,以及癌组织REG4表达量与外周血CEA水平的相关性。分析癌组织REG4表达量与患者术后5年累积总生存率的关系。结果REG4在直肠癌组织中的表达高于癌旁正常组织,差异有统计学意义(P<0.05)。患者癌组织REG4高表达组与低表达组之间性别比例、年龄、BMI、大体类型、分化程度、肿瘤直径及M分期比较,差异均无统计学意义(均P>0.05),高表达组T3~T4期患者、N2期患者占比高于REG4低表达组(均P<0.05)。T分期、N分期是直肠癌组织REG4表达量的独立影响因素(均P<0.05)。直肠癌组织REG4表达量与外周血CEA水平间呈正相关关系(r=0.507,P<0.001)。REG4高表达组患者术后5年累积总生存率低于REG4低表达组,差异有统计学意义(P<0.05)。结论直肠癌组织中REG4表达量高于癌旁正常组织且与外周血CEA水平呈正相关,其与患者T分期、N分期有关,REG4高表达患者术后5年累积总生存率低于低表达者。Objectives To investigate the expression of REG4 in rectal cancer tissue and its relationship with patients’clinico pathological characteristics and prognosis. Methods Clinical data of 98 patients with rectal cancer treated in our hospital be tween January 2010 and January 2013 were collected. The patients’rectal cancer tissue and adjacent normal tissue were re trieved to detect REG4 expression levels. Relationship between REG4 expression and patients’clinicopathological characteristics, peripheral carcinoembryonic antigen (CEA) level, and 5-year cumulative survival were analyzed. Results REG4 expression was significantly higher in the rectal cancer tissue than in the adjacent normal tissue (P < 0.05). There was no significant difference in sex, age, body mass index, degree of differentiation, tumor diameter, and M stage between the high and low expression groups (P > 0.05). The proportion of patients with T 3 ~T 4 /N 2 was significantly higher in the high expression than in the low expression group (P < 0.05). T and N stage were independent factors of REG4 expression in rectal cancer tissue (P < 0.05). There was a sig nificantly positive correlation between REG4 expression and CEA level (r = 0.507, P < 0.001). Five-year survival rate was signifi cantly lower in the high expression group than in the low expression group (P < 0.05). Conclusion REG4 expression was higher in the rectal cancer tissue than in the adjacent normal tissue, and its expression positively correlated with CEA level. REG4 ex pression was related with T and N stage. Patients with high expression of REG4 had worse 5-year cumulative survival than those with low expression.
