循环IGF-1、IGFBP-3和VEGF在非小细胞肺癌中的临床价值及相关性分析

摘　　要：目的探讨循环胰岛素样生长因子-1(IGF-1)、胰岛素样生长因子结合蛋白-3(IGFBP-3)、血管内皮生长因子(VEGF)在非小细胞肺癌(NSCLC)中的临床价值及相关性。方法采用化学发光法检测了30例健康体检者(对照组)及50例NSCLC患者的IGF-1、IGFBP-3、VEGF变化,比较其与患者病理特征之间的差异及3个指标间的相关性。结果NSCLC患者IGFBP-3水平低于对照组,VEGF水平高于对照组(P<0.05),两组间IGF-1水平差异无统计学意义(P=0.754)。鳞癌组VEGF水平明显高于腺癌组(P<0.05),IGF-1、IGFBP-3在两组中差异无统计学意义(P>0.05)。TNM分期越高的患者IGFBP-3水平越低(P=0.038)。VEGF与TNM分期、肿瘤大小、淋巴结转移都有关系(P<0.05),VEGF与肿瘤远处转移有一定的关系(P=0.047)。相关性分析结果显示IGFBP-3与IGF-1呈正相关(γ=0.762,P=0.000),与VEGF呈负相关(γ=-0.358,P=0.011)。结论IGFBP-3在NSCLC中低表达且与TNM分期相关;VEGF在NSCLC中高表达并与患者多项病理特征相关。VEGF与IGFBP-3在循环中的水平呈负相关。Objective To explore the clinical value of the circulating IGF-1,IGFBP-3,and VEGF in non-small cell lung cancer(NSCLC) and their correlation. Methods The changes of IGF-1,IGFBP-3 and VEGF were detected by chemiluminescence in 30 persons undergoing the physical examination and 50 patients with NSCLC.The difference between them and the pathological features of the patients was compared,and the correlation among 3 indexes was studied. Results The IGFBP-3 level in the NSCLC patients was significantly lower than that in the control group,the VEGF level was higher than that in the control group ( P <0.05),and serum IGF-1 level had no statistically significant difference between these two groups ( P =0.754).The VEGF level in the squamous cell carcinoma group was significantly higher than that in the adenocarcinoma group ( P <0.05),the IGF-1 and IGFBP-3 levels had no statistically significant difference between these two groups ( P >0.05).The higher the TNM stage,the lower the IGFBP-3 level ( P =0.038).VEGF had the relation with the TNM stage,tumor size and lymph node metastasis ( P <0.05),and VEGF had a certain relation with the tumor distant metastasis ( P =0.047).The correlation analysis results showed that IGFBP-3 had a positive correlation with IGF-1 (γ=0.762, P =0.000),and negative correlation with VEGF (γ=-0.358, P =0.011). Conclusion IGFBP-3 has low expression in NSCLC and is correlated with TNM stage;VEGF is highly expressed in NSCLC and is correlated with multiple pathological characteristics of the patient.Meanwhile,VEGF is negatively correlated with the level of IGFBP-3 in circulation.
