DNA倍体分析和尿脱落细胞学检查在尿路上皮癌的临床应用价值

摘　　要：目的:分析DNA倍体分析和尿脱落细胞学检查在尿路上皮癌诊断中的应用。方法:选择笔者所在医院2014年3月-2018年12月收治的52例尿路上皮癌患者纳入观察组,另取52例血尿患者纳入对照组,两组均接受DNA倍体分析和尿脱落细胞学检查。结果:观察组尿脱落细胞学检查阳性率为94.23%,高于对照组的11.54%;观察组DNA倍体分析阳性率为98.08%,高于对照组的7.69%,差异有统计学意义(P<0.05)。DNA倍体分析诊断膀胱癌准确率为97.67%,诊断上尿路上皮癌准确率为100%;尿脱落细胞学检查诊断膀胱癌准确率为95.35%,诊断上尿路上皮癌准确率为88.89%。两种诊断方法诊断准确率比较差异无统计学意义(P>0.05)。结论:DNA倍体分析、尿脱落细胞学检查均可为尿路上皮癌的诊断提供支持。Objective:To analyze the application of DNA ploidy analysis and urinary exfoliation cytology in the diagnosis of urothelial carcinoma.Method:A total of 52 patients with urothelial cancer admitted to our hospital from March 2014 to December 2018 were enrolled in the observation group,and 52 patients with hematuria were enrolled in the control group.Both groups received DNA ploidy analysis and cytological examination of urinary exfoliation.Result:The positive rate of urine exfoliative cytology in the observation group was 94.23%,which was higher than 11.54% in the control group,and the positive rate of DNA ploidy analysis in the observation group was 98.08%,which was significantly higher than 7.69% in the control group,the differences were statistically significant(P<0.05).The accuracy of DNA ploidy in the diagnosis of bladder cancer was 97.67%,and the accuracy of the diagnosis of upper urothelial carcinoma was 100%,the accuracy of urine exfoliative cytology in the diagnosis of bladder cancer and epithelial carcinoma was 95.35% and 88.89% respectively,There was no significant difference in diagnostic accuracy between the two methods(P>0.05).Conclusion:DNA ploidy analysis and cytological examination of urinary exfoliation can provide support for the diagnosis of urothelial carcinoma.
