牙周再生术前RCT、术后RCT与未行RCT的重度牙周-牙髓联合病变患者牙周再生效果比较

摘　　要：目的比较牙周再生术前根管治疗术(RCT)、术后RCT与未行RCT的重度牙周-牙髓联合病变患者牙周再生效果。方法回顾性分析西宁市口腔医院收治的45例(49个患牙)重度牙周-牙髓联合病变患者的临床资料,根据治疗方式的不同,分为A组(仅行牙周再生术,未行RCT)12例,B组(牙周再生术后6个月行RCT)15例,C组(牙周再生术前3个月行RCT)18例。比较三组患者牙周再生手术前后牙周骨密度、临床附着丧失(CAL)、探诊深度和咀嚼功能的差异。结果A组牙周再生术后6个月、12个月牙周骨密度灰度值与术前比较,无显著差异(P>0.05)。B组牙周再生术后12个月牙周骨密度较术前显著升高,C组术后6个月、12个月牙周骨密度较术前均显著升高,B、C组术后12个月牙周骨密度较A组均显著升高(P<0.05)。A组牙周再生术后CAL和探诊深度与术前相比,无显著差异(P>0.05)。B组牙周再生术后12个月CAL和探诊深度较术前显著下降,C组术后6个月、12个月CAL和探诊深度较术前均显著下降,B、C组术后12个月CAL和探诊深度较A组均显著下降(P<0.05)。三组患者术后12个月咀嚼功能评分较术前均显著增高(P<0.05),而三组患者术后12个月咀嚼功能评分的组间比较,无显著差异(P>0.05)。结论与术后RCT及未行RCT相比,牙周再生术前行完善的RCT在促进重度牙周-牙髓联合病变患者牙周再生方面具有良好的效果。Objective To compare the periodontal regeneration effects of preoperative root canal therapy(RCT),postoperative RCT and severe combined periodontal-endodontic lesions without RCT.Methods The clinical data of 45 patients(49 affected teeth)with severe combined periodontal endodontic lesions in our hospital were retrospectively analyzed.According to the different methods of treatment,these patients were divided into group A(only received periodontal regeneration,without RCT)12 cases,group B(after 6 months of periodontal regeneration line RCT)15 cases,group C(periodontal regeneration of preoperative 3 months line RCT)18 cases.The differences of periodontal bone density,clinical attachment loss(CAL),probing depth and chewing function between the three groups before and after periodontal regrowth surgery were compared.Results There was no significant difference in the gray value of periodontal bone mineral density between the 6th and 12th months after periodontal regeneration in group A(P>0.05).Periodontal BMD of group B was significantly increased 12 months after periodontal regeneration compared with that before surgery,while that of group C was significantly increased 6 and 12 months after surgery,and that of group B and C was significantly increased 12 months after surgery compared with that of group A(P<0.05).There was no significant difference in CAL and probing depth between group A after periodontal regeneration(P>0.05).At 12 months after periodontal regeneration,CAL and depth of diagnosis in group B were significantly decreased compared with that before surgery.At 6 and 12 months after surgery,CAL and depth of diagnosis in group C were significantly decreased compared with that before surgery;and at 12 months after surgery,CAL and depth of diagnosis in group B and C were significantly decreased compared with that in group A(P<0.05).The chewing function scores of the patients in the three groups at 12 months after surgery were significantly higher than those before surgery(P<0.05),while there was no significant
