血清PG联合胃镜检查在胃癌诊断中的应用效果

摘　　要：目的探讨血清胃蛋白酶原(PG)联合胃镜检查在胃癌诊断中的应用效果。方法选取我院收治的236例胃黏膜病变患者作为观察组,选取同期体检的60例健康人员作为对照组。根据胃镜检查及镜下组织活检结果,将观察组患者分为慢性浅表性胃炎组、慢性萎缩性胃炎组、胃溃疡组、胃癌组。比较各组研究对象的血清PGⅠ、血清PGⅡ水平及血清PGⅠ/PGⅡ值;分析血清PG和胃镜检查单独检测与血清PG检测联合胃镜检查对胃癌的诊断情况。结果观察组的血清PGⅠ水平和PGⅠ/PGⅡ值均低于对照组,PGⅡ水平高于对照组(P<0.05)。各组研究对象的血清PGⅠ、PGⅡ水平和PGⅠ/PGⅡ值与对照组比较,差异均具有统计学意义(P<0.05);胃癌组的血清PGⅠ、PGⅡ水平和PGⅠ/PGⅡ值与其它各组比较,差异均具有统计学意义(P<0.05)。血清PG联合胃镜检查诊断胃癌的灵敏度高于血清PG和胃镜检查单独应用(P<0.05);血清PG联合胃镜检查对胃癌诊断的特异度高于胃镜检查,误诊率低于误胃镜检查(P<0.05)。结论血清PG联合胃镜检查可提高胃癌诊断的灵敏度、特异度,减少误诊的发生,值得临床推广应用。Objective To explore the application effect of serum pepsinogen (PG) combined with gastroscopy in the diagnosis of gastric cancer. Methods A total of 236 patients with gastric mucosal lesions admitted in our hospital were selected as observation group, and 60 healthy persons in the same period were selected as control group. According to the results of gastroscopy and biopsy, the patients in the observation group were divided into chronic superficial gastritis group, chronic atrophic gastritis group, gastric ulcer group and gastric cancer group. The levels of serum PGⅠ and PGⅡ and the value of serum PGⅠ/PGⅡ in each group were compared;the diagnosis of gastric cancer by serum PG, gastroscopy alone and serum PG combined with gastroscopy were analyzed. Results The level of serum PGⅠ and PGⅠ/PGⅡ value in the observation group were lower than those in the control group, and the level of PGⅡ in the observation group was higher than that in the control group (P<0.05). The levels of serum PGⅠ, PGⅡ and PGⅠ/PGⅡ values in each group were significantly different from those in the control group (P<0.05);the levels of serum PGⅠ, PGⅡ and PGⅠ/PGⅡ value in the gastric cancer group were significantly different from those in other groups (P<0.05). The sensitivity of serum PG combined with gastroscopy in the diagnosis of gastric cancer was higher than that of serum PG and gastroscopy alone (P<0.05);the specificity of serum PG combined with gastroscopy in the diagnosis of gastric cancer was higher than that of gastroscopy, and the misdiagnosis rate was lower than that of gastroscopy (P<0.05). Conclusion Serum PG combined with gastroscopy can improve the sensitivity and specificity of the diagnosis in gastric cancer and reduce the occurrence of misdiagnosis, which is worthy of clinical promotion and application.
