摘 要:目的运用活体共聚焦显微镜(IVCM)定量检测睑板腺功能障碍(MGD)患者睫毛根部毛囊内的蠕形螨。方法选取2017年9~11月就诊于华中科技大学同济医学院附属协和医院眼科门诊的连续性MGD患者52例(103只眼)为观察组,年龄与性别相匹配的非MGD患者31例(62只眼)为对照组。所有入组患者均进行裂隙灯检查、IVCM观察睫毛根部毛囊内蠕形螨感染情况,并记录毛囊内蠕形螨数量。运用SPSS23.0软件对两组间蠕形螨的数量进行独立样本t检验分析。结果MGD组蠕形螨阳性率为89.32%,对照组蠕形螨阳性率为62.90%。两组蠕形螨感染阳性率差异有明显统计学意义(P<0.001)。MGD组平均每个眼睑感染的蠕形螨数量(7.28±3.42)只显著高于对照组(4.49±2.43)只,差异有统计学意义(P<0.001)。MGD组平均每个毛囊感染的蠕形螨数量(0.81±0.38)只也显著高于对照组(0.50±0.27)只,差异有统计学意义(P<0.001)。结论IVCM可以有效的对睑板腺功能障碍患者睫毛根部毛囊内的蠕形螨进行无创、定量检测。Objective To quantitatively detect the demodex in eyelash follicles in patients with meibomian gland dysfunction (MGD) using in vivo confocal microscopy (IVCM).Methods Fifty-two consecutive patients (103 eyes) with MGD who visited the ophthalmological outpatient department of Union Hospital affiliated to Tongji Medical College of Huazhong University of Science and Technology from September to November 2017 were selected as the observation group, and 31 non-MGD patients (62 eyes) matched by age and gender were selected as the control group.All enrolled patients underwent slit lamp examination and IVCM to observe the demodex in eyelash follicles.The number of demodex was recorded. SPSS 23.0 software was used to perform independent sample t test analysis of the numbers of demodex in both groups.Results The positive rate of demodex was 89.32% in the observation group,and 62.90% in the control group.The demodex positive rate in two groups had significant difference ( P <0.001). The average number of demodex per eyelid in the observation group (7.28±3.42) was significantly higher than that in the control group (4.49±2.43)( P <0.001).The average number of demodex per hair follicle in the observation group (0.81±0.38) was also significantly higher than that in the control group (0.50±0.27)( P <0.001).Conclusions IVCM is an effective method for noninvasive and quantitative detection of demodex in eyelash follicles in patients with MGD.
