非静脉曲张性上消化道出血血清胃泌素检测及意义

摘　　要：目的　了解非静脉曲张性急性上消化道出血血清胃泌素水平及其临床意义。方法　A组 :急性非静脉曲张性上消化道出血 34例 ;B组 :食管胃底静脉曲张 13例 ;C组 :活动性消化性溃疡 2 9例 ;D组 :慢性胃炎 30例 ;E组 :萎缩性胃炎 11例。采用放免法检测血清胃泌素。结果　A组血清胃泌素为 97.94± 2 2 .75ng/L ,95 %可信限(95 %CI) 92 .0 8～ 10 3.80ng/L ;B组血清胃泌素为 10 2 .87± 2 5 .38ng/L ,95 %CI 89.0 5～ 116 .6 9ng/L ;C组血清胃泌素为5 2 .31± 9.94ng/L ,95 %CI 4 8.70～ 5 5 .94ng/L ;D组血清胃泌素为 35 .15± 11.95ng/L ,95 %CI 30 .88～ 39.4 2ng/L ;E组血清胃泌素为 39.2 7± 13.2 8,95 %CI 33.4 3～ 4 9.11ng/L。A、B两组胃泌素显著高于C、D、E组 (P <0 .0 0 1) ,C组胃泌素也显著高于D、E组胃泌素 (P <0 .0 5 ) ,D、E两组胃泌素相近 (P >0 .0 5 )。结论　胃泌素分泌增多与消化道出血相关。Objective To investigate the changes of gastrin and its clinical significance in patients with non-variceal gastrointestinal bleeding.Methods The serum gastrin was determined with radio-immunoassay (RIA) in thirty-four patients with acute non-variceal gastrointestinal bleeding (Group A); in thirteen patients with gastroesorphageal varices (Group B); in twenty-nine patients with acute peptic ulcer (Group C); in thirty patients with chronic superficial gastritis(Group D); in eleven patients with atrophy gastritis (Group E).Results The level of serum gastrin in Group A was 97.94±22.75ng/L, the 95% confidence interval (95%CI) was 92.08-103.80ng/L; in group B,102.87±25.38,95%CI 89.05-116.69ng/L; in group C, 52.31±9.94ng/L, 95%CI 48.70-55.94ng/L; in group D, 35.15±11.95ng/L, 95%CI 30.88-39.42ng/L; and in group E, 39.27±13.28, 95%CI 33.43-49.11ng/L. Group A and group B vs group C?D?E,P<0.001; group C vs group D?E, P<0.05. Group D vs E, P>0.05.Conclusions The increase of serum gastrin is related to the non-variceal gastrointestinal bleeding.
