摘 要:目的观察不同心脏起搏电极植入后起搏阻抗的变化及其临床意义。方法 对 33例植入心脏起搏器患者的 4 9条电极 ,其中分形涂复电极 2 5条 ,激素释放电极 2 4条 ,用配套体外程控仪测定术后 1、10、2 2、30、90及 180天的起搏阻抗和电压阈值。结果发现分形涂复电极的起搏阻抗逐渐降低伴随相应的电压阈值逐渐升高 ,阻抗低谷及电压峰阈值约在术后 10天左右出现 ,分别为 4 2 6±6 9Ω和 0 95± 0 2 1V ,然后又逐渐恢复 ,术后 30天以后达到其术后 1天的水平并保持至术后 180天。激素释放电极的起搏阻抗及电压阈值始终保持在一个稳定的水平 ,整个随访期无明显波动。结论起搏阻抗和电压阈值均能很好反映起搏电极的电性能以及起搏器安置术后电极 /组织界面处的情况。Objective To investigate the pacing impedance (PI), voltage threshold (VT) variability and clinical significance. Methods 25 Fractally-coated leads (FCL) and 24 Steriod-eluting leads (SEL) were implanted in 33 patients. PI and VT were measured by means of relevant telemetry program at 1st, 10, 22, 30, 90 and 180 days after implantation. Results 1. PI of FCL decreased gradually after implantation with corresponding VT increased. The PI valley or VT peak appeared at 10th day. After that these two parameters recovered day by day and reached at the level of 1th day at 30th day, this phenomenon exist till 180 day after implantation. 2. PI and VT of SEL kept stable and had no significant variability in the whole follow-up period. Conclusion Both PI and VT can reflect the electrical performance of pacing lead and the condition at the lead/tissue interface after the implantation. Combining with these two pacing parameters can help the clinic to program output settings safely in proper time.
