E-钙粘附蛋白,CD44V6在原发性肝细胞癌中表达的研究

摘　　要：目的　 E-钙粘附蛋白 (E- Cad) ,CD4 4 V6在原发性肝细胞癌中的表达及意义。方法　高压锅抗原修复免疫组化技术。结果　正常肝组织 E- Cad表达显著高于癌旁组织和肝癌组织 (P<0 .0 5 ) ,CD4 4 V6仅在肝癌组织中见到表达 ,E- Cad表达与原发性肝细胞癌的包膜是否完整及癌细胞分化程度密切正相关 ,与有无门静脉瘤栓形成密切负相关 (P<0 .0 5 ) ,CD4 4 V6的表达与原发性肝细胞癌有无门静脉瘤栓形成密切正相关 (P<0 .0 1) ,单因素分析显示 E- Cad、CD4 4 V6的表达与原发性肝细胞癌患者的预后相关 (P<0 .0 5 ) ,Cox比例风险模型分析表明 E- Cad、CD4 4 V6、门静脉瘤栓、肝癌细胞分化程度可作为影响预后的指标 (P<0 .0 5 )。结论　 E- Cad和 CD4 4Objective To elucidate the expression and significance of E Cad?CD44V6 in bepatocellular carcinoma(HCC).Methods Immunohistochemistry technigue was used to detect the expression.Results The expression of E Cad in normal liver tissues was significantly higher than it in HCC and interface areas(P<0.05).CD44V6 only was present in HCC tissues.The expression of E Cad in HCC tissues positively correlated with tumor capsule and the differentiation of HCC cells,inversely correlated with tumor embolus in portal vein (P<0.05).The expression of CD44V6 positively correlated with emobolus in portal vein(P<0.01).By log-rank test,it was demonstrated that the expression of E Cad?CD44V6 was correlated with the prognosis(P<0.05).In Cox smultivariate analysis,E Cad?CD44V6?tumor embolus in portal vein?differentiation of HCC cells were regarded as significant prognosis factors(P<0.05).Conclusion E Cad plays an important role in the process of invasion?metastasis and differentiation of HCC.CD44V6 presents the potention of invasion and metastasis of HCC.The expression of E Cad and CD44V6 can be regarded as a parameter to evaluate tumor metastasis and prognosis of HCC.
