集束巨能刀射频治疗肺癌的临床应用

摘　　要：目的探讨集束巨能刀射频治疗肺癌的适应证、治疗效果以及各种并发症的发生和处理。方法在局麻和模拟定位机定位下应用集束巨能刀治疗周围型肺癌14例。结果治疗后患者均有不同程度的发热,3例治疗过程中出现咯血,2例治疗后出现痰中带血。无疼痛、血胸及气胸等并发症发生。治疗后早期肿瘤表现为轻度增大,约1个月后部分肿瘤渐出现空洞样改变。本组中肿瘤无变化者4例,其中1例行穿刺活检,病理发现肿瘤细胞完全坏死,同时大量炎性细胞浸润和肉芽组织生成。结论采用局麻和模拟定位机定位行集束巨能刀治疗肺癌安全可靠,效果肯定,省时省力,费用低。适用于禁忌外科手术的周围型肺癌的治疗。Objective To study the adaptation,therapeutic efficacy,occurrence and management of various complications of assembly-energy scalpel(radiofrequency thermal ablation)in the treatment of lung cancer.Methods The14cases with peripheral type lung cancer were treated via the scalpel under local anesthesia and analog -localization.Results Post -therapeutic fever occurred in almost all the patients while hemoptysis in5.No severe pain,hemothorax,pneumothorax and other complications occured.The size of the tumour in early period after treatment became somewhat bigger,about 1month later the cavum-like change revealed gradually in some tumours.No changes of the tumours appeared in4cases,among whom one was given biopsy pathologically,his tumour cells necrosied thoroughly,infiltration of mass inflammation cells and the formation of granulation tissue were observed under microscope.Conclusion Radio frequency thermal ablation performed under local anesthesia and analog-localization is safe,reliable,time-saving,labor-saving and money-saving.It is an appropriate method for the treatment of peripheral type lung cancer which has operative contraindications.
