摘  要:目的:探讨反流性食管炎(RE)的病理生理学基础。方法:应用比色法和放免法分别测定20例RE和20例无食管炎(NE)者下食管括约肌(LES)局部组织内一氧化氮(NO)、血管活性肠肽(VIP)和P物质(SP)的含量。结果:不同程度RE组LES组织中的NO和VIP含量均明显高于NE组(P<0.01),SP含量则明显低于NE组(P<0.01);不同程度RE组各组间LES组织中的NO、VIP和SP含量差异均有非常显著性意义(P<0.01)。RE的严重程度与NO、VIP含量呈显著正相关(r_(NO)=0.983,P<0.01;r_(VIP)=0.964,P<0.01),与SP呈显著负相关(r_(SP)=-0.966,P<0.01)。NO和VIP含量之间呈显著正相关(r=0.972,P<0.01)。NO与SP含量之间及VIP与SP含量之间均呈显著负相关(r_(NO)=-0.967,P<0.01;r_(VIP、SP)=-0.916,P<0.01)。结论:NO、VIP和SP在RE的发病机制中发挥着重要的病理生理学作用。Purpose: To investigate the pathophysiological features of reflux esophagitis(RE). Methods: The amounts of nitric oxide(NO), rasoactive intestinal polypeptide( VIP) and substance P(SP) were detected lower esophageal sphincter (LES) tissue of 20 patients with RE and 20 cases with no esophagitis (NE). Results: The amounts of NO, VIP and SP within LES tissue in vary degrees of RE group were higher than that of NE group(P<0.01), the amounts of SP within LES tissue in RE group were lower than that of NE group(P<0.01). Significant difference was found among cases with vary degrees of RE themselves(P<0.01). Positive correlation was noted between damage degree of RE and tissue concentration of NO and VIP( rNO = 0.983, P < 0.01; rVIP = 0.964, P < 0.01). While negative correlation was noted between demage degree of RE and tissue concentration of SP( rsp = -0. 966, P<0.01). The amounts of NO and VIP were related positively (r = 0.972, P< 0.01). While the amounts of NO and VIP or VIP and SP related negatively(rNO,sp = -0.967,P<0.01;rVIP,sp= -0.916,P<0.01). Conclusion: There-suits suggested that NO, VIP and SP play an important pathophysiological role in the mechanism of RE.
