BRCA_1蛋白在乳腺癌组织中的表达及其与临床病理关系

摘　　要：目的:探讨 BRCA_1蛋白在乳腺癌组织中的表达以及与乳腺癌临床病理的关系,了解 BR-CA_1蛋白的表达与 P53及 C-erbB_2表达之间的相关性。方法:采用 LDP 免疫组化法对60例乳腺癌,10例乳腺良性疾病患者的石腊标本进行了 BRCA_1蛋白、P53、C-erbB_2的联合检测,结合患者的年龄、家族史、病理组织学分级,腋淋巴结的转移情况,雌、孕激素受体状况等因素进行相关分析。结果:BRCA_1蛋白的阳性表达率在恶性组为61.66%(37/60),在良性组表达为0(0/10)。BRCA_1蛋白的表达绝大部分为浆表达。BRCA_1蛋白的表达与年龄呈负相关(r=-0.295,P<0.05),与家族史呈正相关(r=0.509,P<0.01),BRCA_1蛋白的表达在不同组织学分级中(Ⅰ-Ⅲ)存在差异(P<0.05),随着组织学分级的增加 BRCA_1蛋白阳性表达的程度也增高(r=0.413,P<0.01),BRCA_1蛋白的表达在有无腋淋巴结转移的病例中存在差异(P<0.05),随着腋淋巴结转移数目的增加,BRCA_1蛋白阳性表达的程度也增高(r=0.365,P<0.01)。BRCA_1蛋白的表达与 ER、PR 之间以及与 P53及 CerbB_2表达之间本研究未见其相关性。结论:BRCA_1蛋白的表达与年轻、有家族史,组织学分级高,腋淋巴结转移相关,随着进一步研究的深入,推测 BRCA_1蛋白的检测将作为估计乳腺癌生物学行为和预后的新指标。Objective:To study the relationship between the expression of the BRCA_1 protein and clinico-pathologieal characteristics of breast cancer and the correlation of the expression of the BRCA_1 pro- tein with over-expression of P53 and c-erbB_2 and with co-expression of P53 and c-erbB_2.Methods:The expressions of BRCA_1,P53 and c-erbB2 of breast cancer tissues taken from 60 breast cancer patients as well as 10 benign breast disease patients were examined using immunohistochemical LDP method All tis- sues were taken from formalin fixed and parattin embedded breast tumour.We analysised the correlation of the results with other parameters which contained age of onset,familial history,histological grade,sta- tus of estrogen receptor and progesterone receptor,axillary nodal status and so on.Results:①The ex- pression of BRCA,was 61.66%(37/60)in breast cancer patients and 0(0/10)in benign diseases pa- tients.The protein expressed was mainly located in cytoplasms.②The correlation relationship between age of onset and the expression of BRCA_1 was significant(r=-0.295,P<0.05).The correlation rela- tionship between the familial history and the expression of BRCA_1 was significant(r=0.413,P<0.01) .⑤The higher rate of the expression of BRCA_1 was found in those patients with higher histological grade (P<0.01)and axillary's lymph nodes metastases(P<0.05).The expression of BRCA,had significant correlation with high nuclear grade and axillary's nodal status(r=0.413,P<0.01)(r=0.365,P< 0.01).Conclusion:Young age of onset,familial history,high nuclear grade,and axillary's nodal status were highly associated with expression of BRCA_1 protein.We hypothesized that it might be a new prog- nostic factor in the future.
