摘　　要：目的分析包括多形性腺瘤、Warthin瘤、基底细胞腺瘤等在内涎腺常见良性肿瘤超声造影血流灌注特点,探讨超声造影对涎腺良性肿瘤的鉴别能力及临床应用价值。资料与方法对67例涎腺肿瘤(多形性腺瘤32例、Warthin瘤24例、基底细胞腺瘤11例)患者行超声造影。对造影动态过程进行定性分析,包括增强强度、增强后边界与体积、增强环以及增强区情况等。使用SonoLiver软件获得时间强度曲线(TIC)并进行定量分析,包括上升时间(RT)、达峰时间(TTP)、平均渡越时间(mTT)、峰值强度、上升支斜率及下降支斜率绝对值。结果定性分析结果提示,3种肿瘤仅增强强度差异有统计学意义(P<0.05);定量分析结果提示,多形性腺瘤的峰值强度、上升支斜率、下降支斜率绝对值分别与Warthin瘤和基底细胞腺瘤差异有统计学意义(P<0.05);而Warthin瘤与基底细胞腺瘤间上述参数差异均无统计学意义(P>0.05)。结论超声造影对多形性腺瘤、Warthin瘤的鉴别具有较好的临床应用价值;但对Warthin瘤、基底细胞腺瘤的鉴别能力相对较弱。Purpose To analyze the contrast-enhanced ultrasonic blood perfusion features of common benign tumors in salivary glands,including pleomorphic adenomas,Warthin tumors and basal cell adenomas and to explore the differential ability as well as clinical application value of contrast-enhanced ultrasound(CEUS)to benign tumors in salivary glands.Materials and Methods CEUS was performed to 67 salivary gland tumors(32 pleomorphic adenomas,24 Warthin tumors and 11 basal cell adenomas).Meanwhile,qualitative analysis was carried out to dynamic imaging process,including enhanced intensity,boundary of post-enhancement,enhanced ring,volume of post-enhancement and area of non-enhancement.SonoLiver software was used to obtain the time intensity curves(TIC)as well as for quantitative analysis,including parameters of rise time(RT),time to peak(TTP),mean transit time(mTT)and maximum intensity,and slope rates of ascending curve and absolute values of descending slop rates.Results Qualitative analytical results showed that among the three tumors,only difference of enhanced intensity was of statistical significance(P<0.05).Quantitative analytical results indicated that difference between maximum intensity,slope rates of ascending curve and absolute values of descending slop rates of pleomorphic adenomas and Warthin tumors and basal cell adenomas was of statistical significance(P<0.05).However,difference of the above mentioned parameters between Warthin tumors and basal cell adenomas was of no statistical significance(P>0.05).Conclusion Contrastenhanced ultrasound is of good clinical application value to the identification of pleomorphic adenomas and Warthin tumors,but is of relative poor identification ability to Warthin tumors and basal cell adenomas.
