摘 要:目的探讨PLEKHA5在小细胞肺癌(SCLC)组织中的表达水平及与临床病理特征和预后的关系。方法研究共纳入2012年1月至2017年12月85例初治SCLC患者,收集用于病理诊断的肿瘤组织和癌旁组织,采用实时定量PCR(QPCR)和免疫组化检测上述组织中PLEKHA5的表达水平;入组患者均使用依托泊苷+顺铂(EP)方案进行静脉化疗,分析PLEKHA5表达与临床病理特征、总生存期(OS)和无脑转移生存期(BMFS)的关系。结果QPCR结果显示,SCLC组织中的PLEKHA5 mRNA水平为1.363±0.469,高于癌旁组织的1.016±0.241(P<0.05);免疫组化染色显示,SCLC组织中PLEKHA5的评分为3.86±1.84,高于癌旁组织的0.89±0.51(P<0.05)。PLEKHA5表达与性别、年龄、吸烟史、肿瘤大小、TNM分期和初诊疾病分期均无关(P>0.05),而与远处转移(脑转移)有关(P=0.017)。Kaplan-Meier生存分析表明,PLEKHA5高表达组的中位BMFS为17.8个月,低于低表达组的24.0个月(P<0.05);PLEKHA5高表达组的中位OS为22.8个月,低表达组中位OS未达(>60.0个月),两组比较差异有统计学意义(P<0.05)。结论PLEKHA5在SCLC组织中高表达,其表达水平高与脑转移以及较短的OS和BMFS有关,PLEKHA5是SCLC的不良预后因素。Objective To investigate the expression of PLEKHA5 in small cell lung cancer(SCLC)and its relationship with clinicopathological characteristics and prognosis.Methods From January 2012 to December 2017,cancer tissues and adjacent tissues of 85 patients with SCLC were collected.Quantitative real-time PCR(QPCR)and immunohistochemistry were used to detect the expression of PLEKHA5 in the above tissues.Patients received intravenous chemotherapy with etoposide/cisplatin(EP)regimen.The relationship between the expression of PLEKHA5 and clinicopathologic characteristics,overall survival(OS)and brain metastasis-free survival(BMFS)was analyzed.Results The relative expression of PLEKHA5 mRNA in SCLC tissues was 1.363±0.469,higher than 1.016±0.241 in adjacent tissues(P<0.05).Immunohistochemistry showed that PLEKHA5 score in SCLC tissues was 3.86±1.84,higher than 0.89±0.51 in adjacent tissues(P<0.05).The expression level of PLEKHA5 was not associated with gender,age,smoking history,tumor size,TNM stage and disease stage(P>0.05),but related to distant metastasis(brain metastasis)with statistic significance(P=0.017).Kaplan-Meier analysis showed that the median BMFS of PLEKHA5 high-expression group was 17.8 months,lower than 24.0 months of low-expression group(P<0.05).The median OS of PLEKHA5 high-expression group was 22.8 months,and that of low expression group was not yet reached(>60.0 months),and the median OS of high-expression group was lower than that of low-expression group(P<0.05).Conclusion The expression of PLEKHA5 is up-regualted in SCLC tissues.The high expression of PLEKHA5 is associated with brain metastasis,short OS and BMFS.PLEKHA5 is an adverse prognostic factor of SCLC.
