摘　　要：目的:探讨瘦素及其受体与大肠癌发生、发展、预后的关系。方法:应用RT-PCR及免疫组化方法检测40例正常人、40例大肠腺瘤、60例大肠癌患者术前及术后3月、术后6月血清瘦素的浓度,分析3组间大肠黏膜组织样本血清瘦素浓度,并分析比较大肠癌组织、大肠腺瘤组织及正常组织中瘦素受体含量。结果:瘦素、瘦素受体在大肠癌组织和大肠腺瘤组织中均有表达,在大肠癌组明显高于大肠腺瘤组(P<0.05)。临床晚期较临床早期的患者有更高的表达,且术前和术后的表达存在统计学差异(P<0.05),但术后3月和术后6月的比较无统计学差异(P>0.05)。结论:瘦素及其受体可能与大肠癌发生发展、预后相关,瘦素或其受体拮抗剂可能作为临床治疗大肠癌或改善预后的一种辅助手段。Objective:To investigate the relationship between leptin and its receptors and the occurrence,development and prognosis of colorectal cancer.Methods:Serum leptin levels were measured by RT-PCR and immunohistochemistry in samples from 40 normal persons,40 patients with colorectal adenomas and 60 patients with colorectal cancer before operation and after operation for 3 or 6 months.Serum leptin levels from colorectal mucosa samples in three groups were analyzed and compared with those from colorectal cancer,colorectal adenoma and normal tissues.Results:Leptin and leptin receptors were expressed in colorectal cancer and colorectal adenoma tissues,and the expression levels were significantly higher in colorectal cancer group than colorectal adenoma group and at late clinical stage than early clinical stage(P<0.05),but there were no statistical differences in comparison between the patients at postoperative 3 and 6 months(P>0.05).Conclusion:Leptin and its receptors may be related to the occurrence,development and prognosis of colorectal cancer.Antagonist for leptin or its receptors may be an adjuvant means for clinical treatment or improvement of prognosis of colorectal cancer.
