摘 要:目的评估和比较用醋酸乌利司他(UPA)治疗的子宫肌瘤患者血清和瘤组织中转化生长因子-β3 (TGF-β3)的浓度,并探讨其与年龄和体质指数(BMI)之间的相关性。方法选取2016年1月-2017年12月在该院妇产科就诊的80名子宫肌瘤患者进行回顾性队列研究,按照治疗方法的不同分为两组,其中使用3个月UPA治疗(5 mg/d)者23例为UPA组,未使用UPA者57例为非UPA组,另选取同期在该院门诊随访未确定子宫肌瘤诊断的40例女性作为对照组。收集患者病史一般资料,行实验室检查、生殖器超声扫描,收集血液和组织样本,测量血清和组织中TGF-β3浓度。结果非UPA组和UPA组患者血清和组织中的TGF-β3浓度比较,差异有统计学意义(P<0. 05)。平均TGF-β3血清浓度:非UPA组为(39. 6±30. 08) pg/ml,UPA组为(12. 35±9. 77) pg/ml,对照组为(10. 88±9. 41) pg/ml。平均TGF-β3组织浓度:非UPA组为(165. 36±87. 38) pg/mg,UPA组为(92. 5±56. 74) pg/mg。UPA组患者平均TGF-β3血清和组织浓度显著降低(P<0. 05),TGF-β3浓度与年龄或BMI之间无相关性(P>0. 05)。结论子宫肌瘤患者中血清和组织TGF-β3浓度的降低可能是UPA对子宫肌瘤产生生物学作用的重要环节,其中TGF-β3与UPA的相互作用关系值得进一步深入研究。Objective To evaluate and compare the levels of transforming growth factor-β3(TGF-β3)in serum and tumor tissues of uterine fibroids treated with ulipristal acetate(UPA)instead of horm ones,and correlations between TGF-β3 and age or body mass index(BMI).Methods A retrospective cohort study was conducted on 80 patients with uterine fibroids adm itted to the departm ent of gynaecology and obstetrics of the sixth people's hospital of zhangjiagang from January 2016 to December 2017.The patients were divided into two groups according to different treatment methods Among them,23 cases treated with UPA for 3 months were UPA group,57 patients without UPA therapy were in the non-UPA group,In addition,40 women without confirmed diagnosis of uterine fibroids during the same period of outpatient follow-up in the hospital were selected as the control group;general information of patient history was collected,laboratory tests,genital ultrasound scans,blood and tissue samples were collected,and serum and tissue were measured TGF-β3 concentration.Results The differences in serum and tissue concentrations of TGF-β3 between the non-UPA group and the UPA group were statistically significant(P<0.05).The mean serum levels of TGF-β3:(39.6±30.08)pg/ml in the non-UPA group,(12.35±9.77)pg/ml in the UPA group,and(10.88±9.41)p g/ml in the control group.The average tissue concentration of TGF-β3:non-UPA group was(165.36±87.38)pg/mg and UPA group was(92.5±56.74)pg/mg.The mean serum and tissue concentrations of TGF-β3 in the UPA group were statistically significantly lower P<0.0 5),and there was no statistically significant correlation between TGF-β3 concentration and age or BMI.Conclusion The decrease of serum and tissue TGF-β3 concentration in patients with UFs may be an important part of the biological effects of UPA on UF,and the interaction between TGF-β3 and UPA is worthy of further study.
