摘 要:目的探讨环氧化酶-2(cyclooxygenase-2,COX-2)在食管癌中的表达及其与放射敏感性的关系。方法选择食管癌患者107例,在治疗前采用免疫组化法检测食管癌组织的COX-2表达情况。所有患者都给予根治性放疗方案,记录放疗敏感性并进行相关性分析。结果107例患者的食管癌组织COX-2表达阳性45例,阳性率为42.1%。所有患者都顺利完成放疗,治疗过程中无严重不良反应发生,放疗敏感78例,放疗抗拒29例。放疗抗拒组食管癌组织的COX-2表达阳性率高于放射敏感组,对比差异有统计学意义(P<0.05),Spearman等级相关分析显示COX-2表达与放疗敏感存在显著负相关性(γ=-0.533,P<0.05)。食管癌组织的COX-2表达阳性率与患者淋巴结转移、临床分期与分化程度都有显著相关性(P<0.05)。Logistic回归模型分析显示食管癌组织的分化程度、淋巴结转移与COX表达为影响放疗敏感性的独立危险因素(P<0.05)。结论COX-2在食管癌中呈现高表达状态,与放射敏感性存在一定的相关性,可共同参与调节食管癌的病理发展特征。Objective To investigate the expression of-2(cyclooxygenase-2,COX-2)in esophageal carcinoma and its relationship with radiosensitivity.Methods 107 patients of esophageal cancer were selected.The expression of COX-2 in esophageal cancer tissues were detected by immunohistochemistry before treatment.All patients were given a radical radiotherapy protocol,and the sensitivity and correlation analysis were recorded.Results The positive rate of COX-2 expression in esophageal carcinoma tissues was 45 patients in the 107 patients,and the positive rate were 42.1%.All patients successfully completed the radiotherapy,and there were no serious adverse reactions occurred during the treatment,78 patients were sensitive to radiotherapy and 29 patients were resistant to radiotherapy.The positive rate of COX-2 expression in the radiotherapy group were higher than that of the radiation sensitive group,and compared the difference were statistically significant(P<0.05).The Spearman correlation analysis showed that there were significant negative correlation between the expression of COX-2 and the sensitivity of radiotherapy(γ=-0.533,P<0.05).The positive rate of COX-2 expression in esophageal cancer tissue weres significantly correlated with lymph node metastasis,clinical stage and differentiation degree(P<0.05).Logistic regression analysis showed that the type of differentiation,lymph node metastasis and COX expression were independent risk factors for radiotherapy sensitivity(P<0.05).Conclusion COX-2 is highly expressed in esophageal carcinoma and has certain correlation with radiosensitivity,it can be involved in regulating the pathological characteristics of esophageal cancer.
