血管栓塞治疗与开颅夹闭治疗脑动脉瘤患者的疗效及安全性研究比较

摘　　要：目的血管栓塞治疗对比开颅夹闭治疗脑动脉瘤患者的疗效及安全性研究。方法随机选取脑动脉瘤患者90例,按先后序列号随机分为2组:一组开颅夹闭组(45例),一组血管栓塞组(45例),开颅夹闭组患者接受开颅夹闭治疗,血管栓塞组患者接受血管栓塞治疗,然后统计分析两组患者的临床疗效、GOS评分、住院时间、术后并发症发生情况及复发情况。结果血栓栓塞组患者治疗的优良率[91.1%(41/45)]显著高于开颅夹闭组[80.0%(36/45)](P<0.05),GOS评分显著高于开颅夹闭组(P<0.05),住院时间显著短于开颅夹闭组(P<0.05),术后并发症发生率为6.7%(3/45),显著低于开颅夹闭组[28.9%(13/45)](P<0.05),但两组患者的复发率为2.2%(1/45)和2.2%(1/45),差异不显著(P>0.05)。结论脑动脉瘤患者接受血管栓塞治疗安全性较开颅夹闭高。Objective To study the clinical efficacy and safety of vascular embolization in the treatment of cerebral aneurysms compared with craniotomy and clipping.Methods 90 patients with cerebral aneurysms were randomly selected and randomly divided into the 2 groups according to serial number:A group of craniotomy clipping group(45 cases),a group of vascular embolism group(45 cases),craniotomy clipping group received craniotomy clipping treatment.The patients in the embolization group were treated with embolization,and then the clinical efficacy,GOS score,hospitalization time,complications and recurrence of the 2 groups were analyzed.Results The excellent and good rate of thromboembolism group was 91.1%(41/45)significantly higher than that of craniotomy clipping group 80.0%(36/45)(P<0.05).GOS score was significantly higher than that of craniotomy clipping group(P<0.05),and hospitalization time was significantly shorter than that of craniotomy clipping group(P<0.05).The incidence of postoperative complications was 6.7%(3/45)significantly lower than that of craniotomy clipping group 28.9%(13/45)(P<0.05).However,there was no significant difference in the recurrence rates of 2.2%(1/45)and 2.2%(1/45)between the 2 groups(P>0.05).Conclusion The safety of cerebral aneurysm patients treated with vascular embolization is higher than that of craniotomy clipping.
