多层螺旋CT诊断局灶性机化性肺炎及与周围型肺癌鉴别的价值

摘　　要：目的:探讨多层螺旋CT在局灶性机化性肺炎诊断及与周围型肺癌鉴别中的价值。方法:收集2016年8月至2018年8月收治的40例局灶性机化性肺炎患者和40例周围型肺癌患者的临床资料,分别作为观察组和对照组,两组均进行多层螺旋CT扫描,比较两组多层螺旋CT图像特征、平扫CT值以及增强扫描CT增加值。结果:两组患者在病灶分布位置、毛刺征、与胸膜关系、支气管充气征、空洞、胸膜凹陷、局部胸膜增厚、纵膈淋巴结肿大等方面差异均有统计学意义(P<0.05)。观察组动脉期CT增加值、静脉期CT增加值及总增加值均高于对照组(P<0.05)。结论:多层螺旋CT在诊断局灶性机化性肺炎和与周围型肺癌鉴别中效果明确,值得临床推荐。Objective:To investigate the value of multi-slice spiral CT in the diagnosis of focal organizing pneumonia and the differential diagnosis of peripheral lung cancer.Methods:The clinical data of 40 patients with focal organizing pneumonia and 40 patients with peripheral lung cancer admitted from August 2016 to August 2018 were collected as an observation group and a control group,respectively.Multi-slice spiral CT scans were performed to compare the characteristics of the multi-slice spiral CT images,the plain CT values,and the enhanced CT enhancement values in both groups.Results:There were significant differences in the location of lesions,burr sign,pleural relationship,bronchial inflation sign,cavity,pleural depression,local pleural thickening,mediastinal lymph node enlargement and so on between the two groups(P<0.05).The added value of arterial CT,venous CT added value and total added value were higher in the observation group than in the control group(P<0.05).Conclusion:Multi-slice spiral CT has a definite effect in the diagnosis of focal organizing pneumonia and differential diagnosis of peripheral lung cancer,which is worthy of clinical recommendation.
