CDFI与TCCS联合MRA在脑部疾病后循环缺血病人诊断中的应用价值

摘　　要：目的探讨彩色多普勒血流显像(CDFI)、经颅彩色多普勒超声(TCCS)联合磁共振血管造影(MRA)在脑部疾病后循环缺血(PCI)病人诊断中的应用价值。方法选取我院2015年4月—2017年6月疑似PCI病人104例,均行MRA、CDFI、TCCS检查,并以数字减影血管造影(DSA)诊断结果为金标准,统计比较MRA、CDFI、TCCS单一与三者联合检测的敏感度、特异度、准确度。结果104例疑似病人中,DSA检查结果显示,PCI阳性64例,PCI阴性40例。三者联合检测敏感度(93.75%)、特异度(95.00%)、准确度(94.23%)高于MRA(75.00%、75.00%、75.00%)、CDFI(71.88%、70.00%、71.15%)、TCCS(56.25%、60.00%、57.69%)单一检测,差异均有统计学意义(P<0.05);椎-基底动脉病变检查时,CDFI V1段、V2段病变检出情况较好,TCCS V4段、基底动脉病变检出情况较好,MRA适用于全面评估。结论CDFI、TCCS联合MRA可提高脑部疾病后循环缺血病人诊断特异度、敏感度及准确度,具有较高的临床应用价值。Objective To evaluate the application value of color Doppler flow imaging(CDFI)and transcranial color code sonography(TCCS)combined with magnetic resonance angiography(MRA)in the diagnosis of patients with posterior circulation ischemia after cerebral disease.Methods MRA,CDFI,and TCCS were performed in one hundred and four patients with suspected posterior ischemia.The results of digital subtraction angiography(DSA)were gold standard.The sensitivity,specificity,accuracy of MRA,CDFI,TCCS were compared.Results DSA examination results showed 64 cases with positive results and 40 cases with negative results.The sensitivity,specificity,and accuracy of combined detection were higher than that in single test MRA,CDFI,and TCCS(P<0.05).For vertebrobasilar examination,CDFI V1 segment and V2 segment were detected relatively higher,TCCS V4 segment and basilar artery lesions were detected relatively higher,and MRA was suitable for comprehensive assessment.Conclusion CDFI and TCCS combined with MRA could improve the diagnostic specificity,sensitivity and accuracy of patients with posterior ischemia after cerebral disease.
