HSP90α联合肿瘤标志物水平检测对肝癌患者预后的评估价值

摘　　要：目的探讨HSP90α联合肿瘤标志物水平检测对肝癌患者预后的评估价值。方法以76例肝癌患者作为研究对象,取所有患者的原发癌组织标本(癌症组)与癌旁组织标本(癌旁组),采用免疫组化分析检测HSP90α与AFP阳性情况,随访调查患者的预后生存情况,并进行相关性分析。结果癌症组的HSP90α与AFP阳性率分别为85.5%和96.1%,癌旁组分别为7.9%和6.6%,对比差异有统计学意义(P<0.05)。随访至2018年3月,76例患者中,死亡20例,生存56例,生存率为73.7%,中位生存时间为(28.33±4.19)个月。在76例患者中,HSP90α、AFP阳性患者的生存率都显著低于阴性患者(P<0.05)。Cox回归模型分析显示:淋巴结转移、HSP90α、AFP阳性为影响肝癌患者预后生存的危险因素(P<0.05)。HSP90α预测患者预后生存的敏感性、特异性为83.3%和76.7%,AFP预测的敏感性、特异性为66.7%和100.0%,HSP90α联合AFP预测的敏感性、特异性为96.7%和100.0%。结论HSP90α与AFP在肝癌患者中都呈现高表达水平,与患者预后生存显著相关,两者联合检测可有效预测肝癌患者的预后生存情况。Objective To explore the evaluation of prognostic value of HSP90αcombined with tumor marker level for patients with hepatocellular carcinoma.Methods 76 patients with hepatocellular carcinoma were studied,the primary cancer tissue specimens(cancer group)and para cancerous tissue specimens(paraneoplastic group)were taken from all patients,and the positive expression of HSP90αand AFP were detected by immunohistochemical analysis.Followed up were conducted to investigate the prognosis and survival of patients.Results The positive rates of HSP90 alpha and AFP in the cancer group were 85.5%and 96.1%respectively,and those of paraneoplastic group were 7.9%and 6.6%,respectively,and the difference were statistically significant(P<0.05).Up to now,there were 20 patients died,and 56 patients survived,the survival rate was 73.7%,and the median survival time were(28.33±4.19)months.In the 76 patients,the survival rates of patients with positive HSP90αand AFP were significantly lower than those of negative patients(P<0.05).Cox regression analysis showed that lymph node metastasis,HSP90αand AFP positive were risk factors affecting prognosis and survival of patients with hepatocellular carcinoma(P<0.05).The sensitivity and accuracy of HSP90αto predict the survival of patients were 83.3%and 76.7%,and the sensitivity and specificity of AFP prediction were 66.7%and 100%,and the sensitivity and specificity of HSP90αcombined with AFP were 96.7%and 100%.Conclusion The high expression level of HSP90αand AFP in the patients with liver cancer are highly correlated with the survival of the patients.The combined detection of theHSP90αand AFP can effectively predict the survival of the patients with hepatocellular carcinoma.
