摘  要:目的探讨晚期食管癌患者肿瘤组织中切除修复交叉互补基因1(ERCC1)和β微管蛋白3(TUBB3)基因联合检测对于中晚期食管癌同步放化疗的临床价值。方法选取62例进行同步放化疗晚期食管癌患者,随机分为对照组30例和实验组32例。2组均采用三维适形放疗,对照组采用顺铂联合5-氟尿嘧啶同步化疗,实验组先采用实时荧光定量PCR检测患者肿瘤组织中的ERCC1mRNA、TUBB3mRNA的表达情况,然后依照其表达情况制定个体化治疗方案。比较2组临床总有效率(RR)、疾病进展时间(PFS)、中位生存时间(OS),观察不良反应。结果实验组缓解率显著高于对照组,差异有统计学意义(P<0.05)。实验组恶心呕吐、血红蛋白减少发生率显著低于对照组,差异有统计学意义(P<0.05)。实验组中位无疾病进展时间、总生存时间均长于对照组(P<0.05)。结论ERCC1、TUBB3基因联合检测指导晚期食管癌个体化治疗,可显著提高患者近期疗效,改善预后,降低毒副作用。Objective To investigate the clinical value of combined detection of ERCC1 and tubulin 3(TUBB3)genes in resection and repair of advanced esophageal cancer with concurrent chemoradiotherapy.Methods 62 cases of advanced esophageal cancer were randomly divided into the control group(n=30)and the experimental group(n=32).Both groups received three-dimensional conformal radiotherapy,while the control group received cisplatin combined with 5-fluorouracil concurrent chemoradiotherapy.The experimental group first used real-time fluorescence quantitative PCR to detect the expressions of ERCC1mRNA and TUBB3mRNA in the tumor tissues of the patients,and then developed individualized treatment plans according to their expressions.The total effective rate(RR),time of disease progression(PFS)and median survival time(OS)were compared between the 2 groups.Results The remission rate in the experimental group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).The incidence of nausea and vomiting and decreased hemoglobin in the experimental group was significantly lower than that of the control group(P<0.05).The median disease-free time and total survival time of the experimental group were both better than those of the control group(P<0.05).Conclusion The combined detection of ERCC1 and TUBB3 genes can significantly improve the short-term efficacy,prognosis and toxicity of patients with advanced esophageal cancer.
