摘　　要：目的研究糖链抗原125(CA125)、癌胚抗原(CEA)、C反应蛋白(CRP)联合检测对胃癌的诊断效果。方法选择57例患者为胃癌组,60例胃良性病变患者为良性病变组。比较胃癌组和良性病变组患者的血清CA125、CEA、CRP水平以及阳性检出率,并统计分析胃癌组患者CA125、CEA、CRP单项检测和联合检测的特异性、准确性以及敏感性。结果胃癌组的血清CA125、CEA、CRP水平均明显高于良性病变组(P<0.05);胃癌组的血清CA125、CEA、CRP阳性检出率均明显高于良性病变组(P<0.05)。胃癌患者CA125、CEA、CRP联合检测的特异性、准确性以及敏感性均高于单项检测(P<0.05)。出现淋巴结转移的胃癌患者的血清CRP水平高于未出现转移的患者(P<0.05),肿瘤浸润到浆膜层和肌层的胃癌患者血清CRP水平高于仅浸润至固有层的患者(P<0.05),血清CA125、CEA水平与病理指标无明显的关系(P>0.05)。结论CA125、CEA、CRP联合检测有助于对胃癌患者进行临床诊断,具有较高的应用价值。Objective To study the diagnostic value of CA125,CEA and CRP in gastric cancer.Methods Selected 57 cases of patients with gastric cancer who were treated as gastric cancer group,60 patients with benign gastric lesions were benign lesions.The serum levels of CA125,CEA,CRP and positive detection rate were compared in the gastric cancer group and the benign lesion group,and the specificity,accuracy and sensitivity of the single test and joint detection of CA125,CEA and CRP in the gastric cancer group were recorded.Results The serum levels of CA125,CEA and CRP in the gastric cancer group were significantly higher than benign lesion group(P<0.05);the positive rates of CA125,CEA and CRP in the serum of the gastric cancer group were significantly higher than benign lesion group(P<0.05);The specificity,accuracy and sensitivity of CA125,CEA,CRP and combined detection in gastric cancer patients were higher than single detection(P<0.05);The level of serum CRP in patients with lymph node metastasis was higher than that of patients without metastasis(P<0.05);The level of serum CRP in patients with gastric cancer infiltrating into serous layer and myometrium was higher than patients with only infiltrating lamina(P<0.05).Serum CA125 and CEA levels were not significantly correlated with pathological indicators(P>0.05).Conclusion Combined detection of CA125,CEA and CRP is helpful for clinical diagnosis of gastric cancer patients and has high application value.
