摘 要:目的探讨不同浸润程度卵巢交界性肿瘤(BOT)病理特征与超声指标的相关性。方法选择BOT患者71例作为研究对象,手术病理确诊为卵巢恶性疾病(恶性组)31例,卵巢良性疾病(良性组)40例,记录所有患者的超声特征,判断BOT的浸润性并进行相关性分析。结果与良性组对比,恶性组常规超声多表现为多发病灶,病灶内部有显著血流信号,病灶直径增加;恶性组的卵巢动脉血流峰值速度与血流量低于良性组,阻力指数高于良性组,对比差异都有统计学意义(P<0.05)。在BOT患者中,直线相关性分析显示BOT浸润与卵巢动脉阻力指数、血流信号、病灶多发有显著正相关性(P<0.05),与卵巢动脉阻力指数、血流峰值速度有显著负相关性(P<0.05)。Logistic回归分析显示卵巢动脉阻力指数、血流峰值速度为影响BOT浸润的独立危险因素(P<0.05)。结论彩色多普勒超声在BOT诊断中的应用有很好的影像学特征,不同浸润程度BOT的病理特征与超声指标有很好的相关性,有利于疾病的早期诊断。Objective To investigate the correlation between pathological features and ultrasonographic features of different borderline ovarian borderline tumors(BOT).Methods 71 cases of BOT patients were selected that included 31 cases of malignant ovarian disease(malignant group)and 40 cases of benign ovarian disease(benign group)were diagnosed by the operation and pathology.The sonographic characteristics of all patients were recorded,and the invasion of BOT were judged and were given the correlation analysis.Results Compared with the benign group,the conventional ultrasound in the malignant group were more showed multiple lesions,had significant blood flow signals in the lesion and the diameter of the lesion were increased.The peak velocity and blood flow of the ovarian artery in the malignant group were lower than those of the benign group,and the resistance index were higher than that of the benign group,and the difference were statistically significant(P<0.05).In the BOT patients,the linear correlation analysis showed that there were significant positive correlation between BOT invasion,the ovarian artery resistance index,blood flow signal and multiple lesions(P<0.05),and had significant negative correlation with the index of ovarian artery resistance and the peak velocity of blood flow(P<0.05).Logistic regression analysis showed that ovarian artery resistance index and blood flow peak velocity were independent risk factors for BOT invasion(P<0.05).Conclusion The application of color Doppler ultrasound in the diagnosis of BOT has good imaging features,the pathological features of BOT with different degree of invasion has good correlation with the ultrasonic index,which is beneficial to the early diagnosis of the disease.
