传统高位结扎肠系膜下动脉术与保留左结肠动脉腹腔镜直肠前切除术的可行性对比分析

摘　　要：目的探讨传统高位结扎肠系膜下动脉术与保留左结肠动脉腹腔镜直肠前切除术的可行性。方法随机选取行腹腔镜直肠前切除术的患者共110例作为研究对象,按照数字表法将其随机分为对照组和研究组,各55例。对照组给予传统高位结扎肠系膜下动脉术,研究组给予保留左结肠动脉腹腔镜直肠前切除术,研究观察2组患者的手术指标、随访结果、病理学结果以及血清中IL-6、IL-8、TNF-α的水平。结果研究组的各项手术指标均显著好于对照组(P<0.05);研究组的随访结果显著好于对照组(P<0.05);但研究组的病理学结果较对照组差(P>0.05)。研究组血清中IL-6、IL-8、TNF-α的水平均高于对照组(P<0.05)。结论传统高位结扎肠系膜下动脉术与保留左结肠动脉腹腔镜直肠前切除术均具有一定的临床疗效,保留左结肠动脉腹腔镜直肠前切除术的临床价值更为突出,患者接受程度更高,值得在临床上进一步推广应用。Objective To compare the feasibility of conventional high ligation of the inferior mesenteric artery and the laparoscopic anterior rectal resection with preservation of the left colon artery.Methods 110 patients who were treated with laparoscopic anterior resection of the rectum were randomly selected as the research subjects,according to the digital table method,they were randomly divided into the control group and the study group,55 cases in each group.The control group was given the traditional high ligation of the inferior mesenteric artery,the study group was given laparoscopic anterior rectal resection with preservation of the left colon artery.The surgical indexes,follow-up results,pathological results and the levels of IL-6,IL-8 and TNF-in the serum were observed in the 2 groups.Results All the surgical indexes of the study group were significantly better than that of the control group(P<0.05),and the follow-up results of the study group were significantly better than that of the control group(P<0.05),and the pathological results of the study group were less than the control group(P>0.05),and the level of serum IL-6,IL-8 and TNF-a in the study group was higher than that of the control group(P<0.05).Conclusion The traditional high ligation of the lower mesenteric artery and the preservation of the left colonic artery by laparoscopic anterior resection of the rectum have a certain clinical effect.The clinical value of the laparoscopic anterior rectal resection with preservation of the left colon artery is more prominent,and the patient's acceptance is higher,and it is worthy of further aplication on the bed.
