塑形垫联合头颈肩膜在头颈部肿瘤放疗体位固定中的应用

摘　　要：目的探讨塑形垫联合头颈肩膜固定法进行头颈部体位固定的临床效果。方法随机抽取头颈部肿瘤放疗患者150例进入临床研究,分为3组,每组50例。A组为塑形垫联合头颈肩膜固定组;B组为发泡胶联合头颈肩膜固定组;C组为常规头枕结合头颈肩膜固定组。从前后方向、头脚方向、左右方向、三维角度上分析3组体位固定的差异。结果A组与B组头脚方向线性、前后方向和头脚方向旋转误差无统计学意义。A组与C组比较,各方向线性和头脚方向旋转误差有统计学意义;前后方向和左右方向旋转误差无统计学意义。B组与C组比较,各方向线性和前后及头脚旋转误差有统计学意义,左右方向旋转误差无统计学意义。结论塑形垫与发泡胶和传统使用较广泛的透明头枕相比,具有更适形、重复性更好的特点,且可减小大部分摆位的三维误差,具有临床推广价值。Objective To study the clinical efficacy of plastic pad combined with head and neck shoulder membrane fixation for head and neck position fixation.Methods 150 patients with head and neck tumors were randomly selected for clinical study.Divided into 3 groups with 50 cases in each group.Group A was plastic pad combined with head,neck and shoulder membrane fixation group.Group B was combined with head,neck and shoulder membrane fixation group.Group C was treated with head-pillow combined with head-neck-shoulder membrane fixation.The difference of the 3 groups of postural fixation was analyzed from the front and back directions,head and foot directions,left and right directions and three-dimensional angles.Results There was no significant difference in head-foot rotation errors between group A and group B in head-foot direction,forward-backward direction and head-foot direction.Comparing group A with group C,the rotation errors in the direction of head and foot have statistical significance.There was no significant difference in the rotation errors between the front and back directions and the left and right directions.Comparing group B with group C,the errors of head and foot rotation in all directions were statistically significant,while the errors of left and right directions were not statistically significant.Conclusion The new plastic cushion compared with Styrofoam has advantage in conformal degree and repeatability,and can reduce positioning error,it has further clinical value.
