保留耳大神经束的腮腺区域性切除术治疗腮腺癌的效果观察

摘　　要：目的探讨保留耳大神经束的腮腺区域性切除术治疗腮腺癌的效果。方法选择腮腺癌患者67例,根据手术方法的不同分为观察组37例与对照组30例。观察组给予保留耳大神经束的腮腺区域性切除术治疗,对照组给予传统腮腺切除术,记录与随访患者的预后。结果观察组的术中出血量、手术切口长度、手术时间、解剖面神经分支数都显著少于对照组(P<0.05)。术后7 d观察组的面神经分级显著好于对照组(P<0.05)。术后3个月观察组与对照组的面瘫、皮肤麻木、Frey's综合征、涎瘘等并发症发生率分别为10.8%和46.7%,观察组低于对照组(P<0.05)。观察组与对照组术后1年的复发率分别为2.7%和3.3%,组间差异无统计学意义(P>0.05)。结论保留耳大神经束的腮腺区域性切除术治疗腮腺癌可减轻手术创伤,改善术后面神经功能,减少术后并发症的发生。Objective To investigate the effects of regional resection of the parotid gland that preserves the auricular nerve bundle in the treatment of salivary gland cancer.Methods 67 cases of patients with parotid gland cancer were selected and were divided into the 37 patients in the observation group and 30 patients in the control group according to the different surgical methods.The control group were given conventional parotidectomy,and the observationg group were given the regional resection of parotid gland with preserved large nerve bundle,recorded the prognosis of the patients.Results The intraoperative blood loss,length of surgical incision,operation time,and number of anatomical facial nerve branches in the observation group were significantly lower than those of the control group(P<0.05).The postoperative 7 d of facial nerve grade of the observation group weresignificantly better than that of the control group(P<0.05).The postoperative 1 month of incidences of facial paralysis,skin numbness,Frey's syndrome,and sputum in the observation group and the control group were 10.8%and 46.7%,respectively,and the observation group were lower than the control group(P<0.05).The postoperative 1-year recurrence rate of the observation group and the control group were 2.7%and 3.3%,and there was no significant difference between the 2 groups(P>0.05).Conclusion Resection of the parotid gland with auricular nerve bundle in the treatment of salivary gland cancer can reduce the surgical trauma,improve the facial nerve function after surgery,and reduce the incidence of postoperative complications.
