东方肝康口服液治疗非酒精性脂肪性肝病疗效观察

摘　　要：目的:观察东方肝康口服液治疗非酒精性脂肪性肝病患者的临床疗效及其作用机制。方法:将非酒精性脂肪性肝病患者120例随机分为东方肝康组及易善复组各60例,东方肝康组口服东方肝康口服液10 mL,3次/d;易善复组口服易善复456 mg,3次/d;2组均治疗6个月。检测治疗前后2组患者谷丙转氨酶(ALT)、谷草转氨酶(AST)、r-谷氨酰转肽酶(r-GT)、总胆固醇(TC)、甘油三酯(TG)、游离脂肪酸(FFA)、空腹血糖(FBG)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、血清铁(SI)、铁蛋白(SF)、转铁蛋白(TRF),并采用彩色B超判定脂肪肝程度。结果:东方肝康口服液可改善非酒精性脂肪性肝病患者肝区不适、乏力、食欲不振等症状(P<0.01),可改善患者脂肪肝的影像学表现(P<0.01),而且体质指数(BMI)及血清肝酶水平、TG、TC、FFA较治疗前下降(P<0.01),疗效与易善复相当,在改善主要症状的程度方面优于易善复(P<0.05)。结论:东方肝康口服液治疗脂肪肝有较好的临床疗效,其机理与改善胰岛素抵抗、减轻铁超载有关。Objective:To observe clinical effects of DongFang GanKang oral liquid in the treatment for nonalcoholic fatty liver disease(NAFLD)and its mechanism.Methods:All 120 patients were randomized into the groups of DongFang GanKang oral liquid and polyene phosphatidylcholine capsules,60 cases each group.DongFang GanKang oral liquid group took the oral liquid,10 mL,three times each day;polyene phosphatidylcholine capsules group took the capsules,456 mg,three times each day,Both groups were treated for six months.To detect ALT,AST,r-GT,TC,TG,FFA,FBG,FINS,HOMA-IR,SI,SF and TRF between both groups before and after treating,and assess the degrees of fatty liver using color Doppler B ultrasound.Results:DongFang GanKang oral liquid could improve the symptoms including liver discomfort,lassitude,poor appetite and others of the patients(P<0.01),and the imaging findings of fatty liver(P<0.01),BMI,the levels of TG,TC and FFA reduced compared with before(P<0.01),its clinical effects were equivalent to polyene phosphatidylcholine capsules,and the degrees of improving main symptoms were superior to polyene phosphatidylcholine capsules(P<0.05).Conclusion:DongFang GanKang oral liquid could obtain better effects in the treatment for NAFLD,and its mechanism is related to improving IR and mitigating iron overload.
