贞芪扶正颗粒联合化疗治疗急性白血病疗效观察

摘　　要：目的:观察贞芪扶正颗粒联合化疗治疗急性白血病的临床疗效。方法:将初诊急性白血病患者100例按入院顺序分为试验组和对照组各50例,试验组在联合化疗的同时服用贞芪扶正颗粒,对照组仅给予联合化疗,比较2组患者诱导治疗疗效,不良反应及T淋巴细胞亚群表达,分析试验组患者染色体核型表达及化疗总有效率。结果:总有效率试验组为86.0%,对照组为78.0%,2组疗效相当(P>0.05),试验组不良反应较对照组减少(P<0.05),且化疗后骨髓抑制时间缩短(P<0.05);2组患者T淋巴细胞亚群数量治疗后均有所增加,但试验组CD3^+、CD4^+及CD4^+/CD8^+明显上升,且高于对照组(P<0.05)、In(16)、t(8,21)、t(15,17)、t(9,22)核型总有效率均达到100%,优于亚二倍体及复杂核型(P<0.05)。结论:贞芪扶正颗粒联合化疗可减轻化疗不良反应,有效恢复患者T淋巴细胞亚群数量,提高其免疫功能,对于具有良好预后及靶向药物治疗的染色体核型改变的患者,诱导治疗总有效率优于亚二倍体及复杂核型患者,急性白血病初诊时细胞遗传学改变对化疗疗效及预后有重要意义。Objective:To observe clinical effects of Zhenqi Fuzheng granule combined with chemotherapy in the treatment of acute leukemia.Methods:A total of 100 patients were randomized into the experiment group and the control group according to the visiting order,50 cases each group,the experiment group took Zhenqi Fuzheng granule based on chemotherapy,and the control group accepted chemotherapy,to compare the effects of induction therapy,adverse reaction and the expressions of T lymphocyte subsets between both groups,to analyze total effective rate of chemotherapy and the expressions of chromosome karyotype of the patients in the experiment group.Results:Total effective rate of the experiment group was 86.0%,higher than 78.0%of the control group,the experiment group was equivalent to the control group in clinical effects(P>0.05),adverse reaction of the experiment group became less than the control group(P<0.05),bone marrow arresting time shortened after the chemotherapy(P<0.05);the number of T lymphocyte subsets after treating increased in both groups,but CD3^+,CD4^+and CD4^+/CD8^+of the experiment group rose notably,and higher than these of the control group(P<0.05),total effective rates of In(16),t(8,21),t(15,17)and t(9,22)karyotype reached 100%,superior to hypodiploid and complex karyotype patients(P<0.05).Conclusion:Zhenqi Fuzheng granule joined with chemotherapy could relieve chemotherapy adverse reaction,effectively promote the recovery of T lymphocyte subsets quantity,raise the patients'immunity,and total effective rate of induction therapy for the patients who underwent chromosome karyotype changes,which indicates good prognosis and could be treated with targeted drug,is superior to hypodiploid and complex karyotype patients,cytogenetic changes are significant for clinical effects of chemotherapy and the prognosis at the first visit of acute leukemia.
