中医外治三联疗法综合治疗重度癌性疼痛的临床观察

摘　　要：目的:通过临床随机对照试验观察口服西医止痛药联合中药涂擦同步微波,配合电针三联疗法综合治疗重度癌痛的临床疗效。方法:将90例经病理学明确诊断为恶性肿瘤伴有重度癌性疼痛、中医辨证为寒凝血瘀证的患者,随机分为治疗组和对照组,各45例,两组患者均予以口服奥斯康定40 mg,每12小时一次,对照组以中药止痛酊涂擦同步微波两联疗法作为基础治疗,止痛酊中所含的药物如乳香、川芎等均具有活血化瘀、通络止痛的作用,对气血郁滞经络所致的疼痛有良好的效果,而微波疗法通过加热,使局部温度恒定,可促进止痛酊中各种药物的渗透吸收,加强温经通络、化瘀止痛的作用。治疗组在前面基础治疗上,配合电针即三联疗法,根据患者临床症状、疼痛位置,在辨证论治基础上选取穴位可达到疏通经络、补益气血的作用。治疗7日为1个疗程,治疗2个疗程后观察患者疼痛缓解情况。结果:治疗组总有效率为93.02%,对照组为81.81%,两组比较差异有统计学意义(P<0.05),治疗组在止痛疗效和提高患者生存质量方面,优于对照组,同时治疗组可明显减少口服止痛药剂量,两组比较差异有统计学意义(P<0.05)。结论:中药涂擦同步微波配合电针三联疗法止痛作用较二联疗法更强,可提高患者生活质量、减少口服西药止痛药的剂量。Objective:To explore the clinical curative effect of comprehensive treatment on severe pain by observing the clinical randomized controlled trial of taking oral medicine painkillers and jointly TCM synchronous microwave,combine with triple therapy.Methods:90 cases of malignant tumor patients with severe cancer pain with Chinese medicine syndrome differentiation of blood stasis after the pathological diagnosis were randomly divided into the treatment group and the control group,with 45 cases in each group,two groups of patients were given 40 mg q12 h oral Austin kangding,the control group received traditional Chinese medicine(TCM)pain tincture inunction synchronization of two couplet of microwave therapy as the foundation treatment,the pain tincture contained drugs such as frankincense,rhizoma ligustici wallichii,etc..all had the function of promoting blood circulation to remove blood stasis,which had good effect on the basis of microwave therapy heating,which made the local temperature constant and promote the infiltration absorption of various drugs,strengthen the temperature effect.On the basis of the treatment of the control group,the treatment group received electric acupuncture,namely,the triple therapy,acupoints were selected on the basis of syndrome differentiation and treatment to dredge meridians and replenish qi and blood according to the clinical symptoms and pain location of patients.Treatment of 7 days formed a course of treatment,the pain relief were observed after 2 courses of treatment.Results:The total effective rate of the treatment group was 93.02%,and that of the control group was 81.81%,the difference between the two groups was of statistical significance(P<0.05),the analgesic efficacy and the improvement of the quality of survival of the treatment group were better than those of the control group,the treatment group could significantly reduce the oral pain medication doses at the same time,the difference between the two groups was of statistical significance(P<0.05).Conclusion:Chinese medici
