摘 要:目的分析广州市人乳头瘤病毒(Human papillomavirus,HPV)的基因分型特点及其与宫颈病变的关系。方法选取2015年6月-2018年3月南方医科大学第三附属医院经HPV基因芯片技术检测和病理活检检测的女性816例,根据病理学诊断将受试者分为宫颈癌组(n=48)、癌前病变组(n=213)、宫颈炎症组(n=421)、正常宫颈组(n=134),分析各组HPV感染的基因分型特点及其与宫颈癌的关系。结果816例受检女性共有337例HPV阳性,479例HPV阴性,阳性率为41.30%;共检测出506个HPV基因型,其中高危型HPV 245例,占48.42%,低危型HPV 261例,占51.58%,高危型常见基因型主要为HPV16,占13.44%;正常宫颈组、宫颈炎症组、癌前病变组、宫颈癌组HPV阳性率呈逐渐升高趋势(P<0.05);癌前病变组、宫颈癌组高危型HPV阳性率高于正常宫颈组、宫颈炎症组,且宫颈癌组高于癌前病变组(P<0.05);Logistic回归分析结果显示HPV、高危型HPV与宫颈癌的发生呈正相关(P<0.05)。结论广州市女性高危型HPV感染常见基因型为HPV16、52、58、53、18、33、31,HPV感染、高危型HPV感染与宫颈癌的发生呈正相关。临床上应根据HPV基因分型特点开展防治工作,减少广州市女性宫颈癌发生的风险。OBJECTIVE To analyze the genotyping characteristics of human papillomavirus(HPV)infection in Guangzhou and its relationship with cervical lesions.METHODS Totally 816 women who were detected by HPV gene chip technique and pathological biopsy in the Third Affiliated Hospital of Southern Medical University from Jun.2015 to Mar.2018 were selected.According to the pathological diagnosis,the subjects were divided into cervical cancer group(n=48),precancerous lesion group(n=213),cervicitis group(n=421)and normal cervical group(n=134).The genotyping characteristics of HPV infection in each group and the relationship with cervical cancer were analyzed.RESULTS Among the 816 female subjects,there were 337 cases of HPV positive and 479 cases of HPV negative,and the positive rate was 41.30%.A total of 506 HPV genotypes were detected,including high-risk HPV with 245 cases,accounting for 48.42%,and low-risk HPV with 261 cases,accounting for 51.58%.The major common genotype of high-risk HPV was HPV16,accounting for 13.44%.The positive rates of HPV in the normal cervical group,the cervicitis group,the precancerous lesion group and the cervical cancer group were gradually increased(P<0.05).The positive rates of high-risk HPV in the precancerous lesion group and the cervical cancer group were significantly higher than those in the normal cervical group and the cervicitis group,and the positive rate in the cervical cancer group was significantly higher than that in the precancerous lesion group(P<0.05).The results of Logistic regression analysis showed that HPV and high-risk HPV were positively correlated with the occurrence of cervical cancer(P<0.05).CONCLUSION The common genotypes of high-risk HPV infection in women in Guangzhou were HPV16,52,58,53,18,33 and 31,and the HPV infection and the high-risk HPV infection are positively correlated with the occurrence of cervical cancer.We should take prevention and control measures according to the characteristics of HPV genotyping,to reduce the risk of cervical cancer occurrence in Guang
